Purpose:To report the 3.0T diffusion-weighted MRI findings in alveolar echinococcosis(AE) of the liver and evaluate the potential role of apparent diffusion coefficients(ADCs) in the characterisation of lesions with/without liquefaction nercosis.Materials and methods: There have been 21 patients with 34 hepatic alveolar echinococcosis(HAE) lesions(≥2cm) conformed in Qinghai university affiliated hospital by 3.0 T liver MRI from November 2013 to November 2015,including a free-breathing diffusion-weighed single-shot echo-planar imaging sequence(b-values=800,1000 and 1200s/mm2).Two radiologists jointly assessed the following lesion features: size,location,presence of cystic and/or solid components of the single and calcification(on the CT).The companion of liquefied necrotic lesions as group A,the without liquefied necrotic lesions classified as group B,and in the workstation measuring each lesion ADC values of the marginal zone. All of the measured data will be analysis with SPSS 17.0 software,and contrast the otherness of the ADC numerical value among groups,and get the best b-value.Results: 16 form A,18 form B,34 were identified by the research. ①If the b numerical is 800s/mm2,the ADC numerical of A,B group border respectively were 1.34(1.10,1.61),0.96(0.86,1.22)×10-3mm2/s,the differences between the border of the two groups had statistically significant.The ADC values of solid part and the surrounding liver parenchyma of the group A were 1.56±0.34,1.20±0.26×10-3mm2/s,the ADC values of solid part and the surrounding liver parenchyma of the group B were 1.50±0.25,1.03±0.28×10-3mm2/s, the differences between the two parts of A,B group had no statistical significance.②When b value of 1000s/mm2,the ADC values for the border of two groups respectively were 1.13(0.95,1.21),0.96(0.87,1.03)×10-3mm2/s, the differences between the border of the two groups of nidus had no statistically significant(Z=2.264,P=0.024<0.05).The ADC values of solid part and the surrounding liver parenchyma of the group A were 1.49±0.25,1.05±0.18×10-3mm2/s,the ADC values of solid part and the surrounding liver parenchyma of the group B were 1.39±0.10,0.96±0.20×10-3mm2/s,it has no statistical significance differences for the A,B group.③When b value of 1200s/mm2,the ADC values for the border of two groups respectively were 1.09(0.97,1.28),0.95(0.90,1.02)×10-3mm2/s,there were significant differences between them(Z=2.0374,P=0.042<0.05).The ADC values of solid part and the surrounding liver parenchyma of the group A were 1.41±0.36,0.99±0.23×10-3mm2/s,the ADC values of solid part and the surrounding liver parenchyma of the group B were 1.37±0.25,0.87±0.26×10-3mm2/s, it has no statistical significance differences for the A,B group.summarize: Through Analysis With Compare, ADC numerical of the hepatic was tested with 3.0T MRI scanner, when b-value=800s/ mm2, The influence of blood perfusion was little for the ADC numerical and it will has a higher image quality, the SNR is moderate and the test of ADC numerical was more stabilized and exact.The 3.0 T MRI diffusion weighted imaging sequence(DWI) can clearly distinguish the border of HAE lesion,solid part,and/or liquefaction necrosis,the average ADC values of with necrotic lesions higher than the border without necrotic lesions’ s, so DWI has important value to assess the infiltration proliferative activity of with/without necrotic lesions.
