Objective:To clarify the mechanism and prognosis of Ulinastatin treating early stage severe acute pancreatitis from the perspective of cytokines.Method:Selecting from patients with acute pancreatitis in Qinghai province people’s hospital, we admitted the patients who show clinical indications within 24 hours, with no high blood pressure, diabetes, gastrointestinal cancer, immune diseases or other complications. In the case of getting the patients’ consent, according to Ranson ≥3 points standard [1, 2] and APACHE-Ⅱ score ≥8 points[3], we select a sample of 40 patients with no other complications. These patients are randomly divided into treatment group(ulinastatin combined with somatostatin therapy) and control group(SS). Each group is with 20 cases. At the time of admission, 24 hours after treatment, 3 days after treatment, 7 days after treatment, 14 days after treatment, we draw these patients’ blood. We used ELISA method to detect concentrations of TNF-α, PAF in human serum of the patients in two groups, and then analyzed statistical data.Result: ELISA was used to detect concentrations of TNF-α, PAF in human serum. The statistical data analysis results showed that the effective treatment rate of the control treatment group is lower than the treatment group. This is statistically meaningful(p <0.05). Comparing the concentrations of TNF-α, PAF in human serum of the patients at the first 24 hours, the third and seventh days after treatment, we find that the results showed significant differences(p <0.05). As the treatment goes, the test value of TNF-α is reduced, and the values of patients from the same group at the third and the seventh day of treatment are also of significant differences(p <0.05). As the treatment continues, PAF test value continue to decrease, and it’s lower than that of the control group at the same time period.Conclusion: 1. The results show that Ulinastatin’s treatment to SPA is effective, with the mechanism of improving the systemic inflammatory.2. Serum TNF-α and serum PAF levels can be one of the indicators of dynamic observation of changes in SAP patients.
