| Objective:To investigate the clinical pathological characteristics of gastric cancer between youth and elderly in Xining area, analyze the prognostic factors, in order to provide theoretical basis for the prevention and diagnosis and treatment of gastric cancer in Xining region.Methods : To collection of our hospital from January 2011 to January 2012 were confirmed by gastroscopy and surgery of young people(16~44 years old), 69 cases of gastric carcinoma and the elderly(45~75 years old), 111 cases of gastric cancer, to clinical manifestation, pathological classification and prognosis were retrospectively analysis.Results① Young group gastric cancer is predominant female(58.0%), elderly group of gastric cancer is predominant male(64.0%),two groups of comparative differences have significant statistical significance(P<0.05).② Young patients with 45 cases(65.2%), the Han nationality in the elderly group Han in 74 patients(66.7%) patients, two groups of patients with the Han, Tibetan, followed by less proportion of other nations, are similar between the two groups have no statistical significance(P>0.05).③ Young patients with clinical symptoms of patients in elderly group and abdominal pain is given priority to, above is similar between the two groups have no statistical significance(P>0.05);Elderly group in the patients with loss of appetite, difficulty swallowing, upper gastrointestinal bleeding and anemia symptoms than youth, are similar between the two groups have no statistical significance(P>0.05);Young patient vomiting symptoms than patients in elderly group, there was no statistically significant difference(P> 0.05).④ Young patients in elderly group than in patients with HP infection rate is high, the more similar between the two groups was statistically significant(P < 0.05).⑤ Young patients with family history of gastric cancer incidence in elderly group patients were 18.8%, 9.0%, are similar between the two groups have no statistical significance(P>0.05).⑥ Young patients with gastric cancer patients with elderly group in parts of the whole is given priority to with the distal, youth cardia/the gastroesophageal junction(EGJ) the incidence of gastric cancer was 26. 1%, in the elderly group EGJ incidence of gastric cancer was 45.0%, in the elderly group EGJ gastric cancer incidence of a disease is higher than the young, are similar between the two groups was statistically significant(P<0.05).⑦ Under endoscopy in patients with advanced gastric cancer in parting, two groups of patients were more performance for Borrmann typeⅢ, on the other hand, the elderly group Borrmann typeⅠratio significantly higher than the young, are similar between the two groups was statistically significant(P < 0.05).⑧ Young group compared with the elderly group, the tumor infiltration depth most concentrated in the serous membrane layer, differences between the two groups was statistically significant(P<0.05);Youth see more than elderly group found in the lymph node metastasis, comparing the two groups was statistically significant(P< 0.05).⑨ Young TNM stage focuses on Ⅲ ~ Ⅳ, accounted for 75.4%, and in the elderly group TNM staging focuses on Ⅰ ~ Ⅱ period, accounted for 55.0%, are similar between the two groups have statistical significance(P< 0.05).⑩ Youth rates were 74.62% at 1 year, 3 year survival rate was 44.08%.In the elderly group l year survival rate was 83.03%, the 3 year survival rate was 60.45%.Elderly group in gastric cancer patients 1 year, 3 year survival rate was higher than that of youth, compared to similar between the two groups have statistical significance(P< 0.05).Conclusion① The incidence of gastric cancer of female youth is higher than the male youth; the incidence of gastric cancer of male is higher than female.② Gastric antrum cancer is main of the young group, gastric predilection sites gradually move up with age, the incidence of gastric cardia cancer began toincrease.③ Whether endoscopic macroscopic type, or TNM stage,degree of malignancy and the prognosis of young gastric cancer, is worse than the elderly gastric cancer. |
