Objectives To understand the visual function and life quality of DR patients, to analyze the related factors affecting the visual function and life quality DR patients, and to put the model of self-nursing theory in Orem as a guide to observe the effect of nursing intervention on DR patients of their visual function and life quality.Methods 1 The study of the influencing factors on visual function and life quality of DR patients: 454 patients with diabetic retinopathy were selected in Department of ophthalmology hospital ward from July 2013 to June 2014 in the hospital treatment.Collect the relevant clinical data of patients with DR using the self-designed questionnaire on basic condition of the patients、visual function and life quality scale、Hamilton anxiety scale、adult health self management ability assessment scale、family function scale and social support rating scale to conduct on-site questionnaire survey.Use the SPSS13.0 statistical software for statistical analysis. Utilize the frequency to represent the count data like the general information, visual function and life quality of DR patients;Utilize the(`x ± s) to represent scores of DR patients with visual function life quality;Utilize the t-test or analysis of variance to represent single factor analysis; In the multiple factor analysis, use each effective variable as the independent variable and use the gaining scores in visual function and life quality to conduct multiple linear regression analysis.2 The study of the intervention effect of visual function and life quality in DR patients:Choose 100 DR patients who were informed consent and voluntarily accepted intervention in visual function and life quality scale total scores which were greater than or equal to 45 points in Department of ophthalmology hospital in diabetic retinopathy ward from 2013 July to 2014 June, and according to random distribution, the patients were divided into control group and intervention group with 50 cases in each group.The control group was given conventional nursing; intervention group was given conventional nursing and intervention using Orem self-care nursing model. By assessing visual function and life quality of DR patients before and after intervention for 6 months,compared the differences between the two groups respectively. Use the SPSS13.0statistical software for statistical analysis. Use the chi-square test on the comparation of measurement count data; Use the t-test on the comparation of measurement count data.Results 1 In the 454 cases of hospitalized DR patients, there were 36.1% patients who were in poor visual function and life quality contidions. The indexs which accounted for poor percent in the visual function and life quality were as follows: the poor daily activity limitations that relate to visual(57.9%), the poor adaptation feeling(42.1%), the poor psychological(39.6%), the poor peripheral vision(37.7%), the poor stereoscopic vision (30.6%), the poor social intercourse(21.1%), the poor activities(20.7%), the poor selfcare(8.4%).2 The single factor analysis showed: the factors of overall visual function and QOL in DR: a comparison of patients in terms of urban and rural areas, income per month, visual acuity, history of DM, Hb A1 c, complication, exercise, anxiety, self management, family function, the difference had statistical significance(P < 0.05).3 The multivariate linear regression analysis showed: The dimension of overall visual function and QOL in DR: the main affecting factors were visual acuity, self management level,comorbidity, anxiety, Hb A1 c, which had statistical significance(P < 0.05).4 Using the comprehensive nursing intervention application for 6 months based on self-nursing theory in Orem after the intervention for patients with DR, we found that visual function and life quality contidions of patients in intervention group were better than the control group in the dimensions of daily activity limitations, self-care, activity, psychological and overall visual function and QOL, which had statistical significance(P < 0.05). While, the dimensions of peripheral vision: sensory adaptation, stereoscopic vision, social intercourse had no statistical significance(P > 0.05).Conclusions 1 The patients with DR had a poor visual function and life quality and the main affecting factors were the visual acuity, self management, anxiety, complications,Hb A1 c.2 The comprehensive nursing intervention applications based on self-nursing theory in Orem can improve the visual function and life quality of DR patients effectively.
