Objective: To compare the manipulative reduction Raglan splint external fixation and open reduction and internal fixation for the treatment of proximal humerus(NEER III) fractures, for clinical treatment of proximal humerus fractures provide reference data.Methods: 40 cases of comparative analysis of the patients were from the Department of orthopedics of Affiliated Hospital of Shandong University of Traditional Chinese Medicine from 2012 to 2014 February were treated 03 months. Random packet, wherein the first group 21 cases of manipulative reduction and splint fixation, group B 19 cases of open reduction and internal fixation. Were observed and compared between the two groups in gender, age, injury factors, limb fracture reduction factors, the excellent and good rate, fracture healing time, shoulder activity degree and 4 weeks after treatment, 24 weeks and 1 years of shoulder joint function score(Constant-Murley score), using statistical software for data processing.Results: The two groups were followed up results of open reduction and internal fixation group was better than that of raglan splint group superior fracture reduction, P<0.05. Fractures and bony healing time splint group were significantly shorter than the operation group, the P values were less than 0.05, comparable. Score 4 weeks of shoulder joint function after treatment, open reduction and internal fixation group good to Raglan splint fixation group, the P<0.05. After 24 weeks of treatment in the two groups of shoulder adduction, outreach activities, no significant difference of Constant-Murley score of shoulder joint. One year after treatment of shoulder joint function evaluation of the excellent and good rate were splint external fixation group 90.5%, open reduction and internal fixation group was 89.5%, P>0.05, no significant difference between the two groups.Conclusion: Open reduction and internal fixation in fracture reduction, early functional rehabilitation of manipulative reduction and splint external fixation was superior to shoulder. But in the fractures and bony healing splint group better. Splint group in through the rational function after rehabilitation training, in the activity of shoulder joint and shoulder joint function score had no significant difference with surgical treatment. Manipulative reduction and Raglan splint in treatment of proximal humerus fractures(NEER III), the curative effect is accurate, and it is worth promotion and application.
