Background:The subject mainly disscuss the gastrointestinal dysfunction after acute iscerebral infarction.Gas barrier is one of the important complications of acute ischemic stroke,also is a causative factor of stroke.Risk factors of Stroke such as the wind, fire,phlegm and blood stasis can cause poison.The toxin factor can cause stroke.So the method of relieving detoxification is a good principle for relieving gas impassability and poison type of stroke. Research shows Cajal(interstitial cells of Cajal,ICC) exists in the gastrointestinal tract,Which have the function of gastrointestinal electric pacemaker,It is located in the enteric nervous system(enteric nervous system, ENS) and smooth muscle cells(smooth muscle cell, SMC).Enteric nervous release of the neurotransmitter on the ICC.Again through the ICC’s connection.The effects on smooth muscle can cause gastrointestinal dynamic to change. So gastrointestinal function change after acute ischemic stroke may be related to Cajal in the change of it’s morphology and function.In the application of traditional Chinese medicine, This topic uses rhubarb as medicine intervention.Through this method of relieving detoxification to improve the function of gastrointestinal dysfunction after stroke and to observe the change of Cajal.Objective:1. To observe the pathologic changes of gastrointestinal system in rats under the acute ischemic stroke model.2. To understand ICC’s structure and the pathological changes in the rats ’ gastrointestinal tract under the acute ischemic stroke model, and explore the potential pathogenesis of gastrointestinal dysfunction in acute iscerebral infarction.3. In the application of Traditional Chinese Medicine, This topic uses rhubarb as drug intervention,and reserchs the mechanism of method of relieving detoxification after acute ischemic stroke.4. Discuss the feasibility analysis through the method of relieving detoxification for the treatment of gastrointestinal dysfunction after acute ischemic stroke.Method:This research adopts the line bolt of middle cerebral artery embolism model in rats, SPF Wister 108 male rats for the experiment object.Through relieving detoxification method as the main therapeutic principle. The rhubarb as the main drug intervention. Domperidone as control group.The experimental rats were randomly divided into rhubarb high dose, rhubarb blow-dose, domperidone group, and to establish normal control rats and model rats group rats as control group.We divided into 5 groups,eg. Sham-operated group Model group,high dose of rhubarb, middle dose group of rhubarb,domperidone group,Each group only 6, also respectively for 24 h, 4 d and 7 d rats(after longa score for the standard).We mainly discusse the method of relieving detoxification,and observe the internal changes of Cajal cells in rats’ gastrointestinal.To observe the changeable of the gastric mucosa and Gastrointestinal hormone(substance P and somatostatin) state.To further evaluation on effect of the detoxification method and discusses the inner mechanism of Cajal from the angle of cytological.Result:1To successfully set up middle cerebral artery embolism model in rats. Rhubarb is divided into high-dose and middle dose group. Physiological saline were used to control in the normal group and model group.Daily lavage,after24 h,4d,7d,take gastric tissue successfully, and place in liquid. Timely observation specimens were targeted.2 The pathological observation2.1 Visual inspection: Model group rats ’ gastrointestinal mucous membrane can appear haemorrhage or ulcer point,and have typical food retention performance. 7d group is far more serious than 4d group. Ulcer,bleeding and erosion can occasionally appear in high-dose and middle dose group of rhubarb,the normal group and control group.But from the quantity and degree are less than the model group.2.2 Optical frames observation: Under the optical frames,we can see a large number of inflamematory cells infiltration in model group rats’ gastrointestinal mucous membrane.The mucosa and submucosa marked hyperemia and edema. 7 d group far more serious than 4 d group, Gastrointestinal mucous membrane of high-dose and middle dose group of rhubarb are less serious than model group.To improve the degree of comparison: high-dose group of Rhubarb> middle dose group of rhubarb> Domperidone> Sham-operated group.2.3 Electron microscopy observation: Under electron microscope,,we observe the structure of Cajal.To observe the changeable of nucleus, chromatin,gap junction,cell membrane,mitochondria, organelles, membrane structure.2.4 Immunohistochemical group: The changes of substance P and somatostatin in Gastrointestinal hormone. The results showed as follows: substance P: Model> Domperidone>middle dose group of rhubarb>high-dose group of Rhubarb> Sham-operated group>normal group. Somatostatin: Model>Sham-operated group> Domperidone>middle dose group of rhubarb>high-dose group of Rhubarb > normal group.Conclusion:Gastrointestinal dysfunction is one of the common complications of acute phase in ischemic stroke. Chinese medicine is efficacious about relieving detoxification method on the treatment of gastrointestinal dysfunction after acute phase in ischemic stroke,even evidence about the role of rhubarb. In the experiment,we found that Cajal cells’ morphology and structure have changed,It is suggested that Cajal cells are involved in the pathogens is of functional gastrointestinal disorders.1. Acute ischemic stroke can lead to the pathological changes of gastric mucosal morphology in rats.2. The change of structure and function about ICC can results in gastrointestinal dysfunction under acute ischemic stroke.3. Rhubarb for the treatment of gastrointestinal dysfunction after acute ischemic stroke is effectly.It may be related to improve the structure and function of ICC.4. Relieving detoxification treatments are effective.
