Clinical Observation On Fangjihuangqi Soup’s Protective Effects Of Lung Reexpansion Injury After One Lung Ventilation

Purposes:Research traditional Chinese medicine prescription Fangjiahungqi soup’s protective effects of the collapsed side pulmonary reexpansion injury after one lung ventilation(OLV) during thoracic surgery,and explore the possible protective mechanism,to provide possible treatments of reducing lung ischemia-reperfusion injury and protecting lung tissue ultrastructure.Methods:Select patients undergoing radical operation of esophageal cancer in the combination of traditional Chinese and western medicine hospital of Wuhan city.There were a total of 36 cases and they were randomly divided into three groups(n = 12):group A as oral Fangjihuangqi soup group,group B as oral simple Astragalus soup group,and group C as oral 5% glucose control group.36 cases were all diagnosed of esophageal cancer by fiber esophagoscopy in the clinic before hospitalized.They were actively preparing for surgery after admission, and examination before operation showed that they didn’t have immune, endocrine,neurological and psychiatric diseases.They also didn’t have serious heart,pulmonary,liver and kidney dysfunction,high bloodpressure,coronary heart disease and other complications.The inspection didn’t find obvious signs of tumor invasion around or distant metastasis.36 patients all got pre-medication one week before operation,100 ml / times, twice a day.We got patients’ arterial blood for arterial blood gas analysis at 10 min after anesthesia induction,OLV 0.5 h,1 h,2 h during the operation and at 2h after operation. We also got their peripheral blood 5 ml to serum superoxide dismutase(SOD),malondialdehyde(MDA) concentration by colorimetric method and to detect Polymorphonuclear(PMN) number.At last we compared blood gas index,SOD,MDA activity and PMN count results of each group to determine the efficacy.Results:36 cases were all diagnosed of esophageal cancer by fiber esophagoscopy in the clinic before hospitalized. They were actively preparing for surgery after admission,and examination before operation showed that they didn’t have immune,endocrine, neurological and psychiatric diseases.They also didn’t have serious heart,pulmonary,liver and kidney dysfunction,high blood pressure,coronary heart disease and other complications. The inspection didn’t find obvious signs of tumor invasion around or distant metastasis.All the patients got pre-medication one week before operation.None of them got gastrointestinal tolerance discomfort,and no one withdrew from this clinical study during medication.The anesthesia process was smooth,and there were no obvious anesthesia related complications during and after operation.36 patients recovered well after operation, and there were no operation complications occurred such as pulmonary atelectasis, anastomosis fistula, anastomis narrow,infection of mediastinum etc.We got such statistical results by SPSS 21.0 asfollows:comparing of each time point, blood gas improved more significantly in group A than in group B(P<0.05).Comparing with the control group, blood gas of group A and group B improved significantly(P<0.01);SOD,MDA activity and PMN count:at each time point comparing with the control group,MDA level decreased, SOD activity increased(P<0.01) and the increasing trend of PMN slowed(P<0.01) both in group A and B.In contrast to the simple Astragalus soup, all the situation were improved further by Fangjihuangqi soup(P<0.05).Conclusions:Fangjihuangqi soup and Astragalus soup all have protective effects on lung reexpansion injury after one lung ventilation,and both of them can reduce lung ischemia-reperfusion injury.In contrast to simple Astragalus soup,Fangjihuangqi soup has a more obvious effect on reducing lung tissue damage and has the better protection on lung tissue.
