Clinical Research About Acupoints Injection Combined With Acupuncture For Treating Primary Hemifacial Spasm

Objective:Primary hemifacial spasm refers to unilateral facial muscles paroxysmal,involuntary intermittent twitched as main symptom, while other neurologic examination is normal.The disease can not get rehabilitation without remedy.It is seriously affect the patient’s quality of life and mental health.This research through to observe the change of spasm strength,spasm grade and the EMG F wave after the treatment to examine the efficacy of treatment of Primary hemifacial spasm with acupoints injection compared with common acupuncture.We explored the mechanism of the acupoints injection compared with common acupuncture treatment on Primary hemifacial spasm to provide a new clinical treatment,and provide theoretical basis for this method in clinical promotion. Methods:All the 60 patients come from Wuhan Hospital of integreted Chinese and western Medicine,s acpuncture departmentand.All the patients meet the diagnostic and inclusion criteria of primary hemifacial spasm.All patients were divided into a band of acpoints injection combine with usual acupuncture and a band of traditi- onal acupuncture, according to methods statistical and there are 30 cases in each group.The treatment group:selection of acupoint : Take away YanglLingQuan point, Proximal take Yifeng point. The YanglLingQuan was injected of the mixture of vitamin B1 and vitamin B12 0.5ml and the Yifeng was 0.2ml.After a short rest,received treatment of traditional acupuncture.Regular- acupoints: Taiyang, Yangbai,Cuanzhu,Quanliao, both side of Hegu and Taichong.It should be punctured 30 min every time. The patients were treated one day and off for the other day, a course include five times,two days for rest between the courses, 10 times as one period of treatment.The controlgroup:regularacupoints: Yifeng, Taiyang, Yangbai, Cuanzhu, Quanliao,both side of Hegu and Taichong.It should be punctured 30 min every time and all patients were treated once a day. One period of treatment including 10 times. Rested two days before beginning the next course, six courses altogether. After the treatment, according to Cohen hemifacial spasm strength grading standards and Penn hemifacial spasm frequency of grading standards to evaluate the therapeutic effects by the numbers.To observe the F wave’amplitude simultaneously, time limit and occurrence rate, so as to preliminary discuss the mechanism of this therapy for the treatment of primary hemifacial spasm. result:1.The horizontal of spasm strength and frequentness:There was no significant difference between two groups on the horizontal of spasm strength and frequentness before treatment. Compared with before,the difference was statistically significant of both group(P<0.05).It prove that both of the treatment group and the control groups can improve the patient’s facial spasticity.But the magnitude of improvement was different between two groups, the improvement of the treatment group is greatly.The curative effect of the treatment group is better than the control group. 2.The clinical curative effect evaluation: after symptomatic treatment,30 cases of the treatment group:The efficiency is 66.7% and the total effective rate is 93.3%.the control group:The efficiency is 50% and overall response rate is 86.7%.According to statistic analysis,the efficient of two groups is significant difference(P<0.05),the curative effect of treatment group is more apparent.3.The comparison of electromyography F wave:the two groups make no odds of the F wave parameters before treatment,made no meaningless.After treatment, the F wave amplitude, time limit and ccurrence rate of both groups were decreased,compared with pre-treatment showed a visible difference(P<0.01,P<0.05).Two groups of treatment methods are all effective for primary hemifacial spasm; But there was distinct difference between two group,The amplitude of the treatment group decline obviously than the control group(P<0.05)and time limit and occurrence rate is lower than the control group too(P<0.01,P<0.01). The treatment group showed a therapeutic advantage. Conclusion:1. Acupoints injection combine with common acupuncture has effect on treatment of Primary hemifacial spasm, the common acupuncture can also be therapeutic,while combination therapy is more obvious of clinical efficacy,less the times of treatment,and cure rate is higher.Combination therapy can be used as a new method of treating Primary hemifacial spasm in the future,and worthy of further study and widely promoted. 2. Acupoints injection combine with common acupuncture can reduce facial electromyography F wave amplitude,timelines and occurrence rate.So we inferred mechanism of Primary hemifacial spasm treated by combination theraphy may be due to the promotion of the facial nerve myelin regeneration,inhibitanterior horn cell excitability, thus reducing abnormal excited conduction.
