BackgroundAs a kind of prethrombotic state,the hypercoagulation state is closely related to many diseases, which has attracted more and more attention in clinical work.Complications of thrombosis,a frequent occurrence following artificial arthroplasty, but the relationship between factors and hypercoagulable state is not yet clear.ObjectiveTo investigate the risk factors of hypercoagulable state in patients with arthroplasty. As well as detection of TEG preevaluation on hyercoagulation state and complications of ability.Methods62 patients undergoing total accept arthroplasty operation and the surgery thromboelastography TEG monitoring in the First Affiliated Hospital of Kunming Medical University between December 2013 and December 2014 in this study. According to the results of TEG detection, divided into hypercoagulation group and normal group, comparing the two groups of hypercoagulation state of risk factors and the difference in postoperative complications. Correlation analysis among the risk factors of hypercoagulation state and TEG data.ResultsA total of 29 patients were included in the hypercoagulation group, 33 patients in the normal group.Hypertension,diabetes,intraoperative use of bone cement were independent risk factors of hypercoagulation state, and has significance in multivariate analysis (P<0.05) Age, sex, operation time, anesthesia, BMI value,intraoperative trans- fusion volume, amount of blood loss, use of the tourniquet has no obvious effect on the hypercoagulable state. In addition, the hypercoagulation group hospitalization days and incidence of cardiovascular events was significantly more than the normal group after operation. Hypertension, diabetes mellitus, cardiovascular complications, hospitalization days was significantly positively correlated with MA of TEG. Preoperative hypertension was positively correlated with CI, the results were statistically significant (P<0.05).Conclusion1)Patients with arthroplasty have higher rates of hypercoagulable state, TEG can diagnose hypercoagulability timely and accurately. The Ma and CI numbers of TEG are the most important index to evaluate the risk factors of hypercoagulable state, the detection of TEG maybe helpful on perioperative management and postoperative thrombosis prevention.2)Preoperative hypertension, diabetes mellitus, intraoperative use of bone cement is an independent risk factor of hypercoagulation state, which should be paid more attention to perioperative management. At the same time,the hypercoagulable state may increased cardiovascular complication rate in patients and postoperative length of hospital stay.3)The effect of posto perative anticoagulation therapy in clinical work are significantly,which greatly reduces the incidence of thrombosis.
