VIA/VILI Screening Of Cervical Precancerous Lesions In HIV Infection Women

[Objective] To comparing three kinds of screening tests, and evaluate the values of the three kinds of screening tests for cervical cancer, and finding a economic, effective and feasible screening test for cervical cancer screening with in HIV infection women and in low-resource settings.[Methods] Thinprep cytology test, high-risk Human papilloma virus testing, VIA/VILI and Colposcopy were performed for 300 HIV infection women with cervical precancerous lesions screening from May 2013 to July 2013 in infectious disease hospital outpatient in Yunnan Province. All the women had colposcopic examinations and biopsies were taken any lesion was found. First evaluate was negative by Colposcopy, cytology tip:LSIL+ or ASCUS combination of high-risk Human papilloma virus infection women had second colposcopic examinations and biopsies were taken any lesion was found. Results of histopathologic tests were the final diagnoses as gold standard. To comparing the sensitivity, specificity, positive likelihood the wire, negative likelihood the wire, Youden’s index and the area under the ROC curve with Thinprep cytology test, high-risk Human papilloma virus testing, VIA/VILI,evaluate the diagnosis effect of between three methods.[Results] 73 cases of cervical biopsy, with 19 high level cervical lesions were histologically confirmed including 11 cervical intraepithelial neoplasia grade 2 (CIN2)and 8 cervical intraepithelial neoplasia grade 3(CIN3).The sensitivity, specificity, positive likelihood the wire, negative likelihood the wire, Youden’s index and the area under the ROC curve of VIA/VILI was (63.2%,77.8%,2.84,2.84,0.87, 0.731),of TCT test was (84.2%,57.4%,1.98,1.98,0.28,0.708) and of HPV test was (100%,35.2%,1.54,0,0.352,0.676).[Conclusions] VIA/VILI is a economic, effective and feasible screening test for cervical cancer screening.It can be sensitive to find high level cervical lesions and not easy to leak check cervical lesions.VIA/VILI performed by physicians with standardized training may be a feasible way in cervical intraepithelial neoplasia screening, which can offer to groups of HIV infection and clinical prospect in low-resource settings.
