[Objective]1. To explore expression of serum leptin, NF-κB, TNF-α, IL-6 and IFN-γ on patients with gastric cancer;2. To explore the correlation between nutrition status of patients with gastric cancer and serum leptin, NF-κB, TNF-a, IL-6 and IFN-y;3. To analyze the possible correlation among Leptin, NF-κB, TNF-α, IL-6 and IFN-γ of patients with gastric cancer,and detect correlation factors for malnutrition of patients with gastric cancer; To provide certain theoretical basis for further studies on the pathogenetic mechanism of malnutrition of patients with gastric cancer.[Methods]Collected 40 cases of patients with gastric cancer (gastric cancer group) who accepted treatment in the 3rd Affiliated Hospital of Kunming Medical University from Dec.2013 to Jan.2015. The clinical data of patients were collected including gender, age, BMI, serum albumin, pathological pattern, pathological stage; applied NRS2002 to perform nutritional risk screening for the selected patients with gastric cancer, which were divided into group with nutritional risk and group without nutritional risk.Collected 30 cases of healthy people who accepted physical examination in Physical Examination Center of our hospital (control group). Applied fasting hemospasia at elbow vein in early morning for all research objects and extracted serum and nucleated cells; applied Cytometric Bead Array (CBA) to determine TNF-a, IL-6 and IFN-y level of gastric cancer group and control group; applied enzyme linked immunosorbent assay (ELISA) to determine leptin, NF-κB p65 level of both groups of specimen. Adopted SPSS 17.0 to perform statistic analysis.[Results]1. The research includes 26 cases of male patients with gastric cancer, accounting for 65%; and 14 cases of female patients, accounting for 35%, with ages ranging from 31 to 77 years (median age,66.5 years); 6 cases with BMI<18.5, accounting for 15%; 15 cases with BMI 18.5~20.5, accounting for 37.5%; 19 cases with BMI> 20.5, accounting for 47.5%; 2 cases with ALB<35g/L, accounting for 5%; 38 cases with ALB≥35g/L, accounting for 95%; adopted NRS2002 to perform screening.25 cases with nutritional risk, accounting for 62.5%; 15 cases without nutritional risk, accounting for 37.5%; 32 cases with pathological pattern as adenocarcinoma, accounting for 80%; 3 cases with signet-ring cell carcinoma, accounting for 7.5%; 5 cases adenocarcinoma with signet ring cell carcinoma, accounting for 12.5%; 6 cases in clinical stage Ⅰ, accounting for 15%; 8 cases in clinical stage Ⅱ, accounting for 20%; 24 cases in clinical stage Ⅲ, accounting for 60%; 2 cases in clinical stage Ⅳ, accounting for 5%; the patients with gastric cancer in stage Ⅲ~Ⅳ are significantly more than that in stage Ⅰ~Ⅱ.2. The expression level of TNF-α, IL-6, IFN-γ of gastric cancer group are higher than that of control group. And serum leptin, ALB level and BMI of gastric cancer group are lower than that of control group. The differences are statistically significant (p <0.05); the horizontal differentiation of NF-κB p65 of the two groups are not statistically significant (p>0.05).3. The expression level of TNF-α, IL-6, IFN-γ of gastric cancer group of nutritional risk are higher than that of control group. And serum leptin, ALB level and BMI of gastric cancer group are lower than that of control group. The differences are statistically significant (p<0.05); the horizontal differentiation of NF-κB p65 of the two groups are not statistically significant (p>0.05).4. The expression level of ALB of gastric cancer group under nutritional risk are lower than that of control group. The differences are statistically significant (p< 0.05); The expression level of serum leptin, NF-κB p65, TNF-α, IL-6, IFN-γ and BMI of the two groups are not statistically significant (p>0.05).5. The expression level of serum leptin among patients with gastric cancer of nutritional risk, BMI lower than those without nutritional risk, and the difference is statistically significant (p<0.05); the expression level of TNF-α, IL-6, IFN-γ in the group with nutritional risk is higher than that of the group without nutritional risk. The ALB level is lower than that in the group without nutritional risk. But the differences are not statistical significant (p>0.05); the difference of NF-κB p65 level between the two groups is not statistically significant (p>0.05).6. The expression level of serum leptin, NF-κB p65, TNF-α, IL-6, IFN-γ and ALB of patients with gastric cancer with different genders, pathological patterns and clinical stages and BMI difference are not statistical significant (p>0.05).7. The difference of IFN-y, ALB level for patients with gastric cancer under nutritional risk between different genders are statistically significant (p<0.05), and expression level and BMI differences of serum leptin, NF-κB p65, TNF-α, IL-6 are not statistically significant (p>0.05); the expression level and BMIT differences of serum leptin, NF-κB p65, TNF-α, IL-6, IFN-γ, ALB of patients with gastric cancer under nutritional risk are not statistical significant (p>0.05).8. The IL-6 and TNF-α, IFN-γ among patients with gastric cancer are significant correlated. The correlation coefficients are 0.7371,0.5296 respectively (p<0.05); TNF-a and IFN-y are significant correlated. The correlation coefficient is 0.4191 (p <0.05); The IL-6 and Leptin,TNF-α,IFN-γ among patients with gastric cancer under nutritional risk are significant correlated. The correlation coefficients are-0.400,0.765,0.493respectively (p<0.05); no obvious correlation detected among other factors.[Conclusion]1. The expression level of TNF-α, IL-6, IFN-γ for patients with gastric cancer is higher than that of healthy people, while the expression level of serum leptin is lower than that of healthy people; no obvious correlation detected on expression level of NF-κB p65 between patients with gastric cancer and healthy people;2. The expression level of TNF-α, IL-6, IFN-γ for patients with gastric cancer of nutritional risk is higher than that of healthy people, while the expression level of serum leptin is lower than that of healthy people; no obvious correlation detected on expression level of NF-κB p65 between patients with gastric cancer of nutritional risk and healthy people;3. Serum leptin, TNF-α, IL-6, IFN-γ are related with occurrence of malnutrition with patients with gastric cancer. Serum leptin is closely related to nutritional status of patients with gastric cancer. The aforementioned factors can be used to assist the determination of nutritional status of patients with gastric cancer;4. The IL-6 and TNF-α, IFN-γ among patients with gastric cancer are significant correlated. TNF-α and IFN-γ are significant correlated. The IL-6 and Leptin,TNF-α,IFN-γ among patients with gastric cancer under nutritional risk are significant correlated. No obvious correlation detected among other factors. The occurrence of malnutrition of patients with gastric cancer is mainly resulted from joint participating, interaction biological effect of cytokines and leptin.
