Double Catheter Technique Application In The Large Intracranial Aneurysms

Objective:To evaluate the effectiveness and safety of the double catheter technique for treatment of the large intracranial aneurysms. when the safety and use of the technology choice and what is principles of choice the patient to use double catheter technique in aneurysms.the surgical techniques and the what’s matters should be pay attention.Methods:Retrospective analysis of 72 cases large aneurysm patients treatment and check’s clinical data in neurosurgeon department in the First Affiliated Hospital of Kunming Medical University in January 2012-December 2014 Analysis the density and degree of embolization, The rate of relapse post operation, cerebral vasospasm (CVS) and thrombosis, the change of APTT and PT cause by surgery in using simple coil embolization technique and stent-assisted coiling and balloon-assisted coiling and dual catheter technique for treatment of the large intracranial aneurysms. Summary the advantage of using dual catheter technique in treatment of larger intracranial aneurysms.Results:Aneurysm diameter in 10mm～15mm have26 cases,16mm～20mm have21 patients,21mm～25mm have 17 cases, more than 25mm have 8 cases, Aneurysmal neck≥ 4mm,the peak/neck< 2.whichl2 patients’ aneurysms with simple coil embolization treatment,27 cases aneurysms with stent-assisted coiling embolization,12 cases with balloon-assisted coiling embolization, dual catheter technique used in21 cases large intracranial aneurysms. two cases relapse and should secondary operation. The two relapse cases were both using simple coil embolization, dual catheter technique coil embolization were no relapse in patients,Compare of the density and degree of embolizationone. stent-assisted coiling and balloon-assisted coiling and dual catheter technique have obvious advantage for simple coil embolization. Compare of CVS and thrombosis and the change of APTT and PT. stent-assisted coiling embolization have more risk than others. after 3 month used DSA to check, no relapse patients had fund. After 1 year.l patient had fund relapse,with stent-assisted embolization.after 2 years 1 patient had fund relapse,with dual catheter technique embolization.all aneurysm with dual catheter technique embolization had great curative effect.Conclusion:With dual catheter technique embolization treatment large intracranial aneurysm. the density and degree of embolization is close to stent-assisted coiling and balloon-assisted coiling.With dual catheter technique embolization have such advantage of small surgical damage, fewer complications, can provide safety and effective treatment.could become first choice technique in treating some special large intracranial aneurysms.
