Emergency Embolization Of Intracranial Aneurysms: Analysis Of 20 Cases

Objective:To analyze relationship between status before coiling and early prognosis of patients with intracranial aneurysms, and evaluate the therapeutic effect of emergency endovascular embolization.Methods:20 cases with intracranial aneurysms which underwent emergency interventional treatment from December 2008 to February 2009 was studied, and a follow up for 1 month was performed. A statistical analyse was done to look for relationship between patients' age, Glasgow coma score, Hunt-Hess grade, aneurysm size before coiling and early prognosis after coiling. Chose data belong to groups of early clipping, late clipping, early rebleeding and early cerebral infarction from published papers compared with our 20 cases to evaluate the improvement of early prognosis after emergency endovascular embolization. Results:16(80%) patients had a excellent prognosis(Glasgow outcome score 5) after coiling. There was negative correlation between patients' Hunt-Hess grades before coiling and Glasgow outcome score at 1 month after coiling, In the cases with Hunt-Hess grades from I to III got better early prognosis than that grade IV. There was positive correlation between patients' Glasgow coma score before coiling and Glasgow outcome score at 1 month after coiling. Patients' age and aneurysm size didn't show statistical correlation with early prognosis after coiling. Compared with the early clipping group, emergency endovascular embolization didn't show statistical difference to early prognosis, but was significantly improved comparing with late clipping. The risk of early rebleeding and cerebral infarction was significantly decreased after emergency endovascular embolization.Conclusion:It is negative correlated between patients' Hunt-Hess grades before coiling and early prognosis after coiling, and positive correlated between patients' Glasgow coma score before coiling and early prognosis after coiling. Emergency endovascular embolization can significantly improve early prognosis of patients with ruptured intracranial aneurysms, and decrease the incidence of early complication.
