Abstract/Summary:
Objective: Cervical cancer is one of the common female cancers,and cervical stump cancer,as a special type of cervical cancer,is very rare.That has led to the lack of awareness among most clinicians about it.Patients with cervical stump cancer are often invasive when diagnosed.The recurrence rate after treatment is high,so the patient’s quality of life is poor.To explore survival and prognostic factors for patients who suffered from cervical stump cancer is of great significance for its prognosis.Materials and methods: 56 patients were treated for carcinoma of the cervical stump at The First Hospital of China Medical University from April 2010 to April 2018.The tumor histopathology types were squamous cell carcinoma in 45(80.4%)cases and adenocarcinoma in 11(19.6%)cases.Clinical stage distribution was: Ⅰ(33.9%),Ⅱ(42.9%),Ⅲ(19.6%),ⅣA(3.6%).According to the stages,41(73.2%)cases were treated with radical radiotherapy,24 of 41 patients accepted cisplatin-based concurrent chemoradiotherapy,14 cases performed radiotherapy alone,other 3 patients completed 4 courses of sequential chemotherapy.15(26.8%)patients underwent surgery,8 of the 15 patients accepted adjuvant radiotherapy with or without chemotherapy after surgery.The Kaplan-Meier method,log-rank test,and multivariate cox regression model were used to determine the overall survival and differences between each group and to identify the prognostic factors in all patients with cervical stump cancer.Results: The 1-,3-and 5-year overall survival(OS)rates of the 56 patients were 92.8%,74.5% and 60.6%,respectively.Univariate analysis identified clinical stage(P=0.030),histopathology type(P=0.040),interstitial brachytherapy(P=0.013),and total radiation dose(P=0.045)as prognostic factors.Cox multivariate analysis revealed that clinical stage(HR=8.66,P=0.046)and histopathology type(HR=3.43,P=0.043)are the independent prognostic factors for cervical stump cancer patients.Conciusions: Cervical stump cancer patients have no worse prognosis than those women with an intact uterus in the current treatment model.Adenocarcinoma and high clinical stage were poor prognostic factors for patients with carcinoma of the cervical stump.
