Comparative Of Laparoscopic And Open Radical Cystectomy With Ureteral Abdominal Incision For Bladder Carcinoma

Objective:To compare clinical outcomes of Laparoscopic radical cystectomy(LRC) and open radical cystectomy(ORC) with ureteral abdominal incision for stage T2-4a bladder carcinoma.Methods:We retrospectively analyzed the clinical data of 52 patients with stage T2-4a bladder carcinoma.23 patients received Laparoscopic radical cystectomy with ureteral abdominal incision and 29 patients received open radical cystectomy with ureteral abdominal incision.The operation time,blood loss,recovery time of intestinal function,total postoperative drainage,time of drainage tube removal,hospital stay,postoperative incision healing time,total hospitalization cost,and incidence of complications were compared between the two groups.Results:Mean operation time of LRC group were not statistically different from open groups(P>0.05).Mean blood loss of the LRC group is less than that of open group(113.50±41.46ml、517.31±239.04 ml,P<0.05). Mean total postoperative drainage of the LRC group is less then that of open group(141.51±70.29ml、406.23±313.35 ml,P<0.05). Mean time of drainage tube removel of the LRC group is less then that of open group(93.12±39.84h、136.56±61.44 h, P<0.05).Painkillers application f the LRC group is less then that of open group(22.96±4.01h、36.76±6.30 h,P<0.05).Mean recovery time of intestinal function of the LRC group is shorter than that of open group(57.73±12.45h、96.96±29.04 h, P<0.05). Mean postoperative incision healing time of the LRC group is shorter than that of open group(6.98±0.15 d 、 8.87±1.44 d,P<0.05).Mean hospital stay of the LRC group is shorter than that of open group(8.24±1.33d、11.04±2.32 d, P<0.05). Mean total hospitalization cost of the LRC group is more expensive than that of open group(49724.39±15299.40 yuan 、41052.27±12378.60 yuan, P<0.05).Mean incidence of complications of the LRC group were not statistically different from open groups(P>0.05).Conclusion: Laparoscopic radical cystectomy with ureteral abdominal incision has less blood loss,shorter recover time of intestinal after operation time,less total postoperative drainage,shorter postoperative incision healing time and shorter hospital stay than open radical cystectomy(ORC) with ureteral abdominal incision,but more hospitalization expenses. Operation time and complications were not statistically different between the two groups(P>0.05).
