Objective:A retrospective analysis of the clinical characteristics of the patients with Crohn’s disease was performed in Second Affiliated Hospital of Dalian Medical University from September 2002 to September 2014. Further increase awareness for Crohn’s diseaseMethods:The clinical features,laboratory findings,endoscopic and histological manifestations,X-ray,CT/MRI imaging were investigated retrospectively in 48 CD patients.Results:In 48 cases of Crohn’s disease in hospitalized,48 patients(male 32,female16)with a mean age of(41.4±16.7)years old and a median course of 10.4 months were included in This study. Age distribution from 18 to 85 years old,from20 to 40 years old it was accounted for 37.5%,more than 60 years old accounted for 29.17%.Duration:from1 day to 13 years(median duration of 10.4 months),intestinal lesions in 12 cases(25%),Ileum-colon multiple lesions 26 cases(54.17%),colorectal lesions in 10 cases(20.83%).the most common symptom is abdominal pain for 44 cases(91.67%) In 48 cases of Crohn’s disease, abnormal stools 22 cases(45.83%).CD patients had most common complications with intestinal obstruction, fistulas and perforation, including 14 cases(29.16%) intestinal obstruction, 12 cases(25%) perforation, intestinal manifestations 10cases(20.83%), 8 cases(16.66%) with fistula formation.8cases(12.5%)CD patients misdiagnosed. Laboratory testing: C-reactive protein increased(6 / 26,21.42%),ESR faster(12 / 36,33.33%),stool routine test + occult blood positive(8 /48,16.67%)Hypoalbuminemia(8 / 48,16.67%).10 cases had increased WBC(10 / 48,20.83%) in the active phase or during infectionphase, 40 patients underwent intestinal barium meal X-ray or CT. The results showed ::10 cases of bowel wall thickening(25%), stenosis 18 cases(39.10%), mucosal flattening 16 cases(34.78%), intestinal multiple lesions 8 cases(33.33%), 12 cases of intestinal fistula(30%). 38 cases had colonoscopy results: Crohn’s disease in 25 cases(65.79%) involving the large intestine and ileum. pathology found In 42 cases of Crohn’s disease: Cumulative full thickness intestinal inflammation 20 cases(47.61%),non-caseous necrosis, granulation tissue formation 22 cases(52.38%), which fibrous tissue changes 4 cases(4.76 %), lymphocytes aggregation two cases(2.38%), crypt abscess formation 2 cases(4.76%).Conclusions:the incidence of Crohn’s disease(CD) patients has increased in recent12 years. CD patients men than women in our hospital, the most common age among 20 to 50 years old.But, we have not found a bimodal distribution phenomenon.Many patients with abdominal pain, diarrhea treatment. Crohn’s disease most common occurs in distal ileum and colon adjacent. There are variety of clinical manifestations and lack of specific symptoms. difficult diagnosis lead to easy to delay treatment.Comprehensive judgment Requires a combination of clinical manifestations,radiographic,digestive endoscopy, pathology and tracking data. CD patients mostly take medical treatment. surgery should be performed only in unefficient medical treatment or serious complications. Surgery rate increased with increasing age. Elderly patients were serous than in younger patients and with poor prognosis.
