Objective:To investigate the significance of laparoscopic salpingostomy in the treatment of hydrosalpinx by analyzing the pregnancy outcomes of the hydrosalpinx patients who had been treated by laparoscopic salpingostomy,including natural pregnancy rate, IVF-ET success rate and postoperative pregnancy time.Methods:194selected hydrosalpinx cases from Sir Run Run Shaw Hospital were treated by laparoscopic salpingostomy.Their pregnancy outcomes were investigated, including intrauterine pregnancy rate,ectopic pregnancy rate,IVF-ET success rate and postoperative pregnancy time.Meanwhile, in order to compare the different effects, the hydrosalpinx patients were divided into unilateral/bilateral hydrosalpinx groups and different surgical method groups. intrauterine pregnancyResults:Among those194cases, natural intrauterine pregnancy rate was26.80%, with61.86%of total pregnancy rate,52.06%of intrauterine pregnancy rate,9.79%of ectopic pregnancy rate,3.09%of recurrence of hydrosalpinx rate, respectively. There were no significant differences between pregnancy and unpregnanc group in their age, duration of infertility, the type of infertility and the past pelvic surgery history.Among59IVF-ET cases after the surgery, pregnancy rate was88.14%. There was significant difference between IVF-ET patients with salpingostomy and without surgery in terms of pregnancy rate (P<0.01). There was no significant difference between unilateral and bilateral laparoscopic salpingostomy in terms of postoperative pregnancy outcomes (P>0.05). There was no significant difference of different surgical method in terms of postoperative pregnancy outcomes (P>0.05).50%women had natural pregnancy within5months, and up to88%women had natural pregnancy within one year.Conclusion:Laparoscopic salpingostomy could treat infertility caused by hydrosalpinx. It might improve the natural and IVF-ET pregnancy outcomes of hydrosalpinx patients. Natural pregnancy was suggested as early as possible, or choose IVF-ET if could not have pregnancy after surgery over12months. In clinical setting, it was difficult to decide which surgical procedure should be offered regarding to hydrosalpinx, because its subjective might have different impact on pregnancy outcomes.
