Objective: To evaluate the function of colonic J-pouch coloanal anastomosis after resection for low rectal cancer comparing to straight anastomosis.Methods:A search for articles from1986to December2014was conducted on Pubmed, Medline, Web of science and so on. Randomized controlled trials (RCTs) comparing colonic J-pouches to straight coloanal anastomosis were selected. Meta-analyses were conducted to evaluate the functional outcome of J-pouch coloanal anastomosis vs. straight anastomosis after resection for low rectal cancer.Results:16articles were identified. After one year follow-up, the function results showed that there were significant difference between colonic J-pouch group and straight coloanal anastomosis group in bowel movement per day(OR=0.15,95%CI:0.06~0.34), stool frequency per day (OR=1.75,95%CI:1.23-2.26), medication use(OR=0.469,95%CI:0.219-0.962), pads use(OR=0.41,95%CI:0.21-0.81), urgency(OR=0.27,95%CI:0.15-0.48); however, there were no difference in sensation of incomplete defecation(OR=1.02,95%CI:0.58~1.80), unable to discriminate flatus from feces (OR=0.45,95%CI:0.18-1.12)Conclusions:After one year follow-up, Colonic J-pouch partially improves bowel function of patients got low resection of rectal cancer.
