Clinical Effect Of MIPPO Technology In The Treatment Of Unstable Anterior Pelvic Ring Fractures

Objective:To evaluate the clinical effected of MIPPO technology in the treatment of unstable anterior pelvic ring fractures.Methods:Choose from September 2010 to September 2014, 29 cases with unstable anterior pelvic ring fractures were treated by the same surgeon,Including 15 males and 14 females with an average age of 47.71 years,range from 32 to 65 years.The reason of damage:By falling 15 cases, 12 cases of car accidents, heavy injured two cases.29 patients by Tile classification, B1 type 5 cases, B2 type 7 cases, B3 6 cases, C1 5 cases, C2 type 3 cases, C3 type 3 cases.Merge of 7cases of hypertension,4 cases of Diabetes, two cases of coronary heart disease,traumatic brain injury, pleural effusion, 18 cases of systemic multiple fractures.The patients underwent surgery within a mean of 6.21 days,range from 4 to 8days.All patients were retrospectively analyzed, according to the grouping operation way,MIPPO groups and traditional llioinguinal group.Collect the patient’s date of surgery interval, duration of surgery, blood loss, postoperative drainage, healing time, load time.Patients discharged from hospital after 1, 3, 6, 12, 24 months review pelvic X-ray to understand the fracture healing,.The fracture situation according to the Matta grades and postoperative recovery according to Majeed score.Results:A total of 29 consecutive patients were evaluated.MIPPO group: surgical duration(76.69?21.21min), intraoperative blood loss(140.63?17.15 ml),postoperative drainage(66.16?9.45ml), incision length(7.13?1.09cm), fracture healing time(7.94?1.12w), complete load time(10.31?1.30w); traditional group: duration of surgery(102.30?23.23min), intraoperative blood loss(171.77?28.46ml), postoperative drainage(113.77?15.18), incision length(10.19?1.18cm), fracture healing time(9.31?1.49w), full weight bearing time(11.46?1.56 w).Comparison of the two groups datas in operative time, blood loss, postoperative drainage, incision length, fracture healing time and full time load, P <0.05,the difference was statistically significant.According to Matta evaluation criteria, MIPPO were excellent in 8 cases, good in 5 cases, moderate in 2 cases, and poor in 1 case, good rate of 81.25%; traditional group excellent in 8 cases, good in 4 cases, moderate in 1case, the difference between 0 cases excellent 92.31 %.According to Majeed score 100 points, MIPPO 7cases were excellent, good in 7 cases,moderate in 2 cases and poor in 0case, good rate of 87.5%; traditional 6 cases were excellent, good in five cases, moderate in 1 case, differential 1 cases, fine rate of 84.6%.Conclusion:In general, MIPPO group than traditional with small incision,simple operation, less bleeding, shorter operative time and other characteristics,In the aspect of anterior pelvic fracture reduction, fixation, and postoperative recovery have similar efficacy.Mastery of the operation time and certain skills in the treatment of anterior unstable pelvic fractures,MIPPO can replace the traditional open reduction method.
