Retrospective Analysis Of The Prognosis Of Simple Hematuria Type Purpura Nephritis In Children

Objective Through the retrospective analysis of clinical data, the paper reports the prognosis of simple hematuria type purpura nephritis in children(henoch- schonlein purpura nephritis, HSPN) and analyzes relevance between prognosis and sexes, ages, degrees of blood in the urineand and the difference between Chinese traditional medicine group and Chinese traditional medicine group combined with immunosuppressor group.Methods The children with simple hematuria type purpura nephritis in kidney disease ward of the first affiliated hospital of Henan college of traditional Chinese medicine in In November 2003 to December 2012 was retrospectively studied. All cases meet the following conditions:The age is older than or equal to three years old,and younger than eighteen years old;Conform to the simple hematuria type HSPN diagnostic criteria(With reference to the purpura nephritis diagnostic criteria presented by pediatrics branch of Chinese medical association kidney disease committee in 2008).Made survey form and recorded the followling information:sex,age,clinical characteristics,kidney pathological conditions, syndrome differrentiation type of Traditional Chinese medicine,the use of traditional Chinese medicine,the usage of immunosuppressant, and follow-up conditions by telephone. According to the patient’s urine and kidney function, based on the standard put forward by Counahan and HSPN evidence-based guidelines for diagnosis and treatment developed by the kidney disease committee of Chinese medical association in 2008,combined with the actual situation of children, the prognostic judgment is divided into A, B, C, D, E, F six grades.Use SPSS 19.0 software to analyze date.If the data after inspection was in accordance with normal distribution, Statistical description would be x ± s,If not, would be M(P25 ~ P75).Difference ofprognosis between different sexes, ages, degrees of blood in the urine at the beginning or two different treatment groups were analyzed by chi-square or rank and inspection first.The meaningful factors would be analyzed by means of Logistic regression analysis.Results This study followes up 135 cases children in accordance with the set of conditions. Followed up for 2.6~10 years, the Shortest course of the disease is 2 weeks, the longest is more than 10 years, no one occurred death, the accelerated nephritis or renal dysfunction, 12 cases occurred proteinuria, 1 case occurred kidney disease level proteinuria in the 7th months of the duration. The proportion of urine normal is on the rise, Follow-up of 1 year,2 year,5 year, the proportion of prognosis in grade A respectively accounted for54.89%,69.35% and 78.79%,the proportion of prognosis in grade B and D is on a falling trend. Due to a better, worse or fluctuant condition of individual patients, the proportion of prognosis in grade C and E of increased slightly.Logistic regression analysis indicated that the degree of blood in the urine, age and sex is not factors affect the prognosis of 1 year, 2 years, 5years. The difference of 1 or 2 years’ prognosis is without statistical significance between Chinese traditional medicine group and Chinese traditional medicine group combined with immunosuppressor group(p<0.05).Conclusion The vast majority of simple hematuria type HSPN in children has a good prognosis of 1 year, 2 years, 5years. The sex,age or degree of blood in the urine at the beginning of the disease has nothing to do with the prognosis of 1, 2, 5 years; The difference of 1 or 2 years’ prognosis is without statistical significance between Chinese traditional medicine group and Chinese traditional medicine group combined with immunosuppressor group.
