Objective: Total hip arthroplasty periprosthetic fracture into two intraoperative and postoperative periprosthetic fracture, fixed type of bone cement into biotypes and two, the paper mainly discusses the application of biological prosthesis elderly whole hip replacement and the reasons for the pathogenesis of postoperative periprosthetic fractures; analysis of biological total hip arthroplasty in patients with preoperative normal circumstances, such as gender, age, body mass index, type of disease and preoperative comorbidities such as prosthesis Compare biotypes total hip arthroplasty patients preoperative and postoperative blood, inflammatory markers, bone biochemical markers, bone mineral density and X-ray changes; related fracture incidence by around biotypes total hip arthroplasty after concurrent investigate the cause of periprosthetic fractures, specifically related risk factors, provide a guarantee for a further increase in total hip arthroplasty surgical safety and reduce surgical complications in patients, to better improve the quality of life of older persons.Methods:This study was collected in March 2013-2014 October, the Affiliated Hospital of Shandong University of Traditional Chinese joint orthopedic doctor and receive medical information biotypes total hip replacement surgery in 42 cases of elderly patients, 42 patients in 18 cases concurrent fractures around the prosthesis, these 18 cases as fracture group, did not fracture group 24 cases, the establishment of Excel database, the first descriptive statistics, and then use SPSS17.0 variables that may lead to fractures around the prosthesis after logistic regression analysis, screening and related factors cause periprosthetic fractures.Results: Two groups in gender, body mass index, bone biochemical markers were significant differences(P <0.05); Preoperative and postoperative red blood cell count, hemoglobin, hematocrit, and white blood cell count was no significant difference(P> 0.05);two groups of patients ESR, CRP value was no significant difference(P> 0.05); compared with the group not fracture, fracture group osteocalcin levels increased, its level was negatively correlated with age, body mass index, ESR and CRP was positively correlated with Ca, P and ALP was no significant difference(P> 0.05); the two groups in operative time, blood loss, drainage and bed time indicators, the difference was not statistically significant(P > 0.05); preoperative and postoperative hip scores six weeks was not statistically significant(P> 0.05), but did not fracture group after 3 months, 6 months after surgery in December and were hip score formation and fracture group significant differences; multivariate Logistic regression analysis: Women, obesity, osteoporosis, trauma and hip dysplasia is caused by factors surrounding the prosthesis after fracture.Conclusion: Elderly biotypes around total hip arthroplasty postoperative fracture prosthesis reasons: women, obesity and bone biochemical markers(alkaline phosphatase, osteocalcin) and other causes bone loss in patients with osteoporosis factors, including osteoporosis is the ultimate cause; patients with underlying diseases(such as cardiovascular disease), history of trauma and hip dysplasia is the cause of postoperative fractures around the prosthesis; and anemia, infection and blood loss, drainage how much, operative time, length of time in bed is not around cause postoperative prosthesis fracture.
