Research Purpose: The purpose of this research is to examine the differential clinical therapeutic efficacy on traumatic dislocation of patella between surgical treatment and conservative treatment based on the statistics analysis of the clinical treatment of traumatic dislocation of patella.Research Method: This paper uses clinical observation as the main research method and collects the sample of 40 patients who has suffered from traumatic dislocation of patella and was treated in the Luoyang Orthopedics Hospital from March 2013 to December 2014. Based on the difference of treatment approach, the entire sample was divided into two groups: the surgical treatment group and the conservative treatment approach. The surgical group is comprised of 10 male patients and 12 female patients who were under the treatment of soft tissue balance operation(retinaculum patellae laterale dissolution and retinaculum patellae mediale tightening operation) or multimodality operation(retinaculum patellae laterale dissolution, retinaculum patellae mediale tightening operation and patellar tendon ingression); The conservative group is comprised of 9 male patients and 9 female patients who are under the treatment of cast immobilization of the wounded limb in the acute stage and flexion and extension exercise protected by brace in the recovery stage. Each group is also divided into two internal group based on patients’ ages(≤ 18 or >18). This paper will evaluate the therapeutic efficacy based on the statistical measurement of Q angle, postoperative knee function Insall evaluation standard and knee joints Kujala evaluation standard. Research results:The surgical group included 13 patients under 18 years old(6 males and 7 females) and 9 patients over 18 years old(4 males and 5 females). Q angle measurement: For the whole group( 12.6°±2.3°),For patients less than 18(12.4°±2.4°),For patients over 18(13.3°±2.1°);Postoperative knee function Insall evaluation:For the whole group(Excellent:15,Good:6,Fair:1, The percentage of excellent and good is 95.4%), For patients less than 18(Excellent:10,Good:3, The percentage of excellent and good is 100%),For patients over 18(Excellent:6,Good:2,Fair:1, The percentage of excellent and good is 88.9%);Knee joints Kujala evaluation: For the whole group(83.6±4.1 points), For patients less than 18(84.4±3.8points), For patients over 18(79.9±3.3points).The conservative group included 10 patients under 18 years old(4 males and 6females) and 8 patients over 18 years old(5 males and 3 females). Q angle measurement: For the whole group(16.1°±2.8°),For patients less than 18(14.9°±2.7°),For patients over 18(16.9°±2.9°);Postoperative knee function Insall evaluation:For the whole group(Excellent:7,Good:4,Fair:4,Bad:3, The percentage of excellent and good is 61.1%), For patients less than 18(Excellent:4,Good:3, Fair:2,Bad:1, The percentage of excellent and good is 70%),For patients over 18(Excellent:3,Good:1, Fair:2,Bad:2, The percentage of excellent and good is 50%);Knee joints Kujala evaluation: For the whole group(71.6±3.2points), For patients less than 18(73.9±3.5points), For patients over 18(70.7±3.6points).Since all the P values for the Q angle measurement, the postoperative knee function Insall evaluation and the knee joints Kujala evaluation are less than 0.05, therefore the research results are statistically signigicant. Conclusion:Based on the statistical results of this paper, the l clinical therapeutic efficacy on traumatic dislocation of patella of the Surgical Group are clearly better than the Conservative Group. And this study also shows that the sooner the patients begin the surgical operation, the better the effects of the surgical peration will be better.
