ObjectiveIn order to explore the superiority and effectiveness of establishing the airway bypass through bronchoscopic radio frequency catheter lung volume reduction in the sheep model of the pulmonary emphysema.Methods12 female sheep aged 6 months were randomly divided into Group A, Group B and Group C, and emphysema model were established by means of the papain administration. The airway bypass was constructed in Group A through bronchoscopic radio frequency catheter, besides, stent placement was put to use. Fibrin gel was injected into bronchi to carry out target bronchial tube blockage with lung volume reduction surgery in Group B. Fibrobrochoscope and medical treatments purely were applied in Group C. The TLC, RV, RV/TLC and FEV1.0 of each sheep was measured and compared before treatment, and 24 hours, 4 weeks and 8 weeks after treatment; chest CT of Group A was examined regularly. All the sheep were executed 8 weeks after treatment for pathological examination.ResultsPulmonary emphysema model were successfully copied in 12 sheep. Reduced lung volume of the treatment area and atelectasis were shown on the chest CT of Group A 4 weeks and 8 weeks after operation. There were significant differences in TLC levels(Ftime=1001.335,p < 0.05), RV levels(Ftime=2602.382,P < 0.05), RV/TLC levels(Ftime=477.172,P < 0.05) and FEV1.0 levels(Ftime= 6133.743,P<0.05), and in therapeutic effectiveness(Fgroup=72.478,P<0.05;Fgroup=107.066,P<0.05; Fgroup=115.774,P<0.05; Fgroup=72.478,P<0.05). The means and time of treatment were interactive(Ftime*group=294.141, P < 0.05; Ftime*group=692.907, P < 0.05; Ftime*group=152.914, P < 0.05; Ftime*group=1601.741,P<0.05). Pathological examination showed that alveolar atelectatic area in treated area,emphysema in no treatment area and granulocyte and lymphocyte infiltration in alveolar atelectatic area in Group A and B. Alveolar tissue emphysema was observed in Group C.ConclusionEstablishing the airway bypass through bronchoscopic radio frequency catheter lung volume reduction is effective and superior in medical treatment of sheep model of pulmonary emphysema.
