Objective To analyze the incidence of premature rupture of membranes in large sample, a research about pregnant women,in order to help understand the characteristics of the disease and provide a basis for clinic managements.Methods From February 1, 2014 to February 1, 2015 in Beijing Military General Hospital, there were 2544 cases pregnant women. Get the basic information about them,such as: residence, age, height, education, pregnancy production history. Record the situation during pregnancy,such as: smoking or passive smoking,with or without urogenital system infection, complications of pregnancy(including hypertension,diabetes, thyroid dysfunction) and so on.According to various observed indicators,all population was put into different groups, all cases were analysiseds retrospectively and compared the differences of the morbidity of PROM between different groups,chi-square test was used to compare the incidence between different groups in order to find whether the difference has statistical significance.If there was the statistically significant differences between the groups of multi layered( three or more), then we compared the incidence of PROM between every layer in group.for one-way orderly hierarchical data,Linear-by-linear Association chi-square test was used; for disorderly multi layered data, Bonterroni testwas used.Results1. Overall incidence of PROM : In 2544 cases of pregnant women, PROM ccurred in392 women, the incidence rate was 15.4%.PPROM ccurred in 103 women,t PROM ccurred in 289 women,the incidence rate were 4.0% and 11.4%.2. the differences of the morbidity of PROM in each groupwere as follows:(1)In age group : According to the age, the population is divided into four groups,comparisons were made between the four groups, The probability of PROM occurredwere different(P <0.05);(2)Grouped by level of education : From low degree to the highly educated : high school or less 、college / university degree and graduate degree or above, comparisons were made between the three groups, The probability of PROM were different(P<0.05);(3)Grouped by smoking during pregnancy : the incidence rate in smoking or passive smoking group was significantly higher than no smoking or passive smoking group(P<0.05);(4)Grouped by the times of abortions : According to the number of the times of abortions, the population was divided into three groups, Among three groups, The incidence rate of PROM was different(P<0.05);(5)Grouped by the incidence of lower genital tract infection: the incidence rate in no lower genital tract infection during pregnancy group was significantly lower than the PROM during pregnancy which had lower genital tract infection group(P<0.05);(6)Grouped by with or without urinary tract infection during pregnancy: without urinary tract infection, asymptomatic bacteriuria during pregnancy / asymptomatic urinary tract infection and symptomatic of urinary tract infection during pregnancy, between three groups, The incidence rate of PROM were different(P<0.05);(7)Grouped by diabetes during pregnancy : The incidence rate of PROM in No diabetes mellitus was significantly lower than diabetes mellitus group(P <0.05);(8)Grouped by residence, height, history of childbirth, pregnancy hypertension, thyroid function: The incidence rate of PROM between groups was the same(P >0.05);3. Statistical analysis about multi- layered( more than three groups) group :(1)Statistical analysis about disorderly multi layered data : grouping by Education : the incidence rate in high school or less group was significantly higher than university /college degree and graduate degree or above group(P<α’);but in the last two groups,The incidence rate of PROM between groups was the same(P>α’);grouping by with or without urinary tract infection during pregnancy:without urinary tract infection,asymptomatic bacteriuria during pregnancy / asymptomatic urinary tract infection andsymptomatic of urinary tract infection during pregnancy(P<α’), but in first two groups,The incidence rate of PROM between groups were the same(P>α’).(2)According to the incidence of urinary tract infection during pregnancy groups: the incidence rate in asymptomatic bacteriuria during pregnancy / asymptomatic urinary tract infection and symptomatic of urinary tract infection during pregnancy was significantly higher than those patients without urinary tract infection( P <α ’), but in first two groups, The incidence rate of PROM between groups was the same(P>α’).Conclusion1. Aged pregnant women, low education level, smoking or passive smoking during pregnancy, multiple miscarriages, genitourinary infections, taking vitamins compound preparations for relatively long time during pregnancy,complications such as diabetes or other fators may be associated with elevated risk of PROM.2. It is avocated that pregnancy should be at the right age, to avoid smoking and passive smoking, multiple abortions, and to strengthen prenatal care and management,to prevent and treat urinary tract infections, to control gestational diabetes during pregnancy is expected to reduce the incidence of complications.
