The DRGs system and prepaid system has achieved some success abroad, China also began to learn from foreign experience with national conditions and gradually implement a single DRGs system.Objective:This study of hospital fee structure、Factors affecting hospitalization expenses of appendicitis employee health insurance patient from January 2009 to November 2013 in a top three hospital of Dalian city, Taking analysis of income and expenditure also. Provide evidences of controlling medical costs unreasonable growth and improving the rational utilization of health resources. Provide a theoretical basis and practical tools to implement effective government action further. Provide a positive reference of formulate specific cost control measures for Hospital decision makers.Methods:To understand the relevant policy control hospital costs abroad and composition of hospital costs by literature search, then to sum up factors and the main measures to control the costs; The continuous variables by mean, standard deviation and so on to take general descriptive analysis; The categorical variables by Frequency, composition ratio to take general descriptive analysis; The correlation analysis by Pearson correlation test; The difference Analysis by T test, analysis of variance; Build the prediction equation of disease by Stepwise linear regression prediction model, then find out the factors of hospital costs and the strength of the relevant factors.Results:Linear upward trend in hospital costs of 5 years; The constituent ratio of treatment costs, special treatment costs, checkups fee appear downward trend; The constituent ratio of inspection fees, blood costs, material costs, large inspection fees, other costs appear upward trend; Inspection fees and major overhaul costs rise significantly accelerate. The main factors affecting the hospitalization expenses appendicitis patients according to effect size followed by:blood transfusion, length of hospital stay, service and retirements, the big check, other diagnostic; There is no statistical significance of age, type of health insurance, medicare supplement, the conceited proportion; Medicare pay and pocket costs appear upward trend; The single disease of appendicitis insurance indicator development is too low.Conclusion:Strengthen medicare actuary, develop dynamic and reasonable payment standard; Strengthen supervision of hospital, control expenses to increase unreasonably; Strengthen government responsibility, Consummate the medical service compensation mechanism.
