Objective: To evaluate the safety, efficacy and tolerability of the high-frequency chest wall in the patients with respiratory disease.Methods: 121 cases of initial diagnosed as chronic obstructive pulmonary disease,pneumonia, bronchial asthma, bronchiectasis were selected in the Second Affiliated Hospital of Dalian Medical University during June 2014 to December 2014. These patients were randomly divided into HFCWO group, percussion on back group and the control group. After patients in hospital were given intravenous application of antimicrobial agents, oral expectorant drugs and support therapy.HFCWO group(43cases) were given HFCWO and conventional therapy. Percussion on back group(37cases) were given percussion and conventional therapy. The control group(41 cases)were given conventional therapy. Recorded groups before and after treatment in patients with changes in heart rate, breathing, blood pressure, pulse oxygen saturation.Used Likert scale to assess HFCWO group and percussion on back group in tolerance of treatment. Compared the clinical data of three groups of patients(cough score,amount of phlegm,sputum turnaround time, length of hospital stay, WBC, ESR, hs CRP and PCT). Compared the COPD patients,s SGRQ scores. All date was analyzed by SPSS 20.0 statistical analysis software, P<0.05 was considered statistically significant.Results:1.Tolerance analysis: HFCWO treatment group, 40 case(93.02%)were well tolerated, 3 case(6.98%) were hardly toleranced.The percussion on back group 33case(89.19%)were well tolerated, while 4 case(10.81%) were hardly toleranced. Therewere no difference(P=0.835).2.In the process of each treatment and after treatment has no adverse events(hypoxemia, arrhythmia, hemodynamic changes, etc.). No complications appeared.Each group before and after treatment in patients with heart rate, blood pressure,respiration, pulse oxygen had no significant statistical difference.3.Symptom improvement analysis: Three groups of treatment of cough and expectoration after scores were lower, compared with before treatment, P<0.05, there were statistically significant. HFCWO group after treatment compared with the other groups, there were significant difference(P<0.05). In the group of HFCWO and percussion on back group, the amount of expectoration drainage were statistically significant(P<0.05). After treatment, they were increased than the control(P<0.05).The control group before and after treatment had no statistical significance(P>0.05).HFCWO group in sputum turnaround time and hospitalization days were less than control(P<0.05).4.Inflammation index analysis: After treatment the WBC, ESR, hs CRP in the three groups were decreased, compared with before treatment, there were statistical significance(P<0.05). Comparison between groups of WBC, HFCWO group decreased compared with the control group, there were statistical significance(P=0.037).Procalcitonin in three groups before and after treatment were not statistically significant(P>0.05).5.Compared each group before and after the treatment of patients in COPD patients,s SGRQ scores. Before treatment, the SGRQ scores of the three groups were not statistically significant(P>0.05). After the treatment, HFCWO group, percussion on back group and control group mean were 11.93±9.33, 18.68±6.78, 20.24±11.14.There was difference between the HFCWO group and control group.There was difference between the HFCWO group and percussion on back group(P<0.05). There was no difference between the percussion on back group and control group(P=0.635).Conclusion:1. HFCWO can be security and well tolerated by most patients. It can be used as a routine treatment of sputum symptoms of respiratory system disease.2. HFCWO can effectively promote expectoration, improve clinical symptoms of respiratory system disease,and reduce hospitalization time.
