Objective:To explore the clinical pathological characteristics and treatment of colorectal stromal tumors through retrospective analysis of diagnosis and treatment on49cases of colorectal stromal tumor. Methods:The clinical pathology and follow-up data of49patients with colorectal stromal tumors were retrospectively analyzed in The First Affiliated Hospital, College of Medicine, ZheJiang University, from December2000to September2009. including clinical data、immunohistochemical features、risk classification of malignant tumor、 follow-up of patients and survival analysis, etc. Results:The median age of patients was54years, There is no significant difference of the incidence rate between male and female (49%v.s51%, P>0.05). Colorectal stromal tumor were usually occult in onset and lack of characteristic clinical symptoms, Main complain involved changes of defecation habit、fecal property, abdominal discomfort and bloody stool.Pathology examination under light microscope showed more for spindle cells, beam or weave pattern; immunohistochemical staining showed high CD117and CD34positive expression rate. the results of follow-up showed that recurrence rates of high risk patients with complete resection lesions were significantly higher (P=0.018), disease-free survival time were significantly shorter (P=0.011) than that in other patients. Conclusion:Immunohistochemical staining was a reliable way to identify colorectal stromal tumor. A complete resection surgery was the best; Local resection was preferred in extremely low and low risk patients; radical resection was suggested in intermediate risk patients. The prognosis of high risk patients with surgery alone was poorer, which should be given a complete resection surgery combined imatinib therapy etc. comprehensive treatment. Regular physical examination, early diagnosis and periodic review were the important methods for colorectal stromal tumor patients who obtained long-term disease-free survival time.
