Aim:For colorectal carcinomas with liver metastases (CRCLM), surgery is believed to provide a longer survival. However, it is still controversial whether to do staged surgery or simultaneous resection for such patients.This study focuses on the evaluation of clinical outcome, safety and effectiveness of laparoscopic resection of colorectal carcinoma with simultaneous resection of liver metastases by either laparoscopy or open surgery and the clinical outcome of laparoscopic versus open liver metastases resection.Method:A retrospective review of clinical data was carried out between May2011and September2014at Zhejiang University affiliated Sir Run Run Shaw Hospital for20cases who underwent simultaneous laparoscopic resection for colorectal carcinoma with liver metastases (TL group) and for16cases who underwent simultaneous resection of colorectal carcinoma by laparoscopy first and then had open surgery for liver metastases resection (AL group). Clinical data included operative method, operative time, intra-operative bleeding, time to first flatus, post operative indwelling abdominal drains evaluation, length of hospital stay, post operative complications and so on.Result:In the totally laparoscopic group, the indwelling abdominal drain indexes (for total number of days) was significantly reduced (P<0.05). In both groups, intra-operative bleeding, operative time, time to first flatus, length of hospital stay post operation were not significant (P>0.05). There was no serious post operative complications or death related to surgery in both groups. Post operative survival was similar in both groups.Conclusion:Laparoscopic simultaneous resection of colorectal carcinoma with liver metastases is a safe and effective method. The operative conditions were similar in both totally laparoscopic simultaneous resection of colorectal carcinoma with liver metastases (TL group)and simultaneous laparoscopic resection of colorectal carcinoma with open surgery resection for liver metastases (AL group). But, the length of hospital stay and retention of indwelling abdominal drain time was shorter in the totally laparoscopic group.
