Comparison Of Two Kinds Of Anterior Cervical Operation Treatment Of Single Level Cervical Spindylotic Myelopathy

Objective: To discuss the influence of two kinds of anterior cervical surgeryfor single segmental cervical spondylosis patients prognosis.Methods: Review of2012July to2014June,55patients with singlesegmental cervical spondylosis,divided into A, B two groups. GroupA (Anterior cervical discectomy and interbody fusion with placement ofallograft bone pad and an anterior plate system) in43patients, including male25cases, female18cases, age34-68years old, average age is51.4yearsold; group B (Anterior cervical discectomy and interbody fusion withplacement of autogenous iliac bone and an anterior plate system) in12patients among them, male7cases, female5cases; age42-71year old, averageage is57.7years old; The clinical efficacy was evaluated using the operationtime,bleeding amount,complications,cervical fusion,Height of anterior bodrerand Height of posterior bodrer (HAB,HPB), cervicalcurvature change (D value, Cobb angle), Steel plate andimplant position, mJOA score and the situation of quality of life (using theSF-36scale) for comparison.Results: all patients were followed up for9-18months, average12months, Compare A group with B group, operation time, bleedingamount, HAB and HPB, D value and the change of the Cobb angle hadstatistical significance (P <0.05); Vertebral fusion rate of A group and Bgroup, mJOA scores had no statistical significance (P>0.05); The two groupswere no implant or (and) plate position change;7cases in group B patientsappear iliac donor site pain; according to SF-36scale analysis, to evaluate thelife quality of two groups of patients, educational level, occupation, medicalcost are the main factors that influence the operation mode selection.Conclusion:1.Fusion rate of two kinds of anterior cervical operation equivalent insingle segmental cervical spondylosis vertebral treatment，Group B canincrease the operation time and bleeding volume during operation;2．B group compared with A group, HAB, HPB, D value, Cobb anglechange of the former is obvious; HAB, HPB, D value and Cobb angle oftwo groups have larger amplitude within3months; After3months,HAB, HPB, D value and Cobb angle of two groups are stable;3.In a certain period of time, cervical fusion rate,HAB and HPB decreased,thechange of D value and the Cobb angle of two kinds of operation is no clearcorrelation with the score of mJOA, but the long-term correlation remains to be further studied;4．Both allogeneic bone pad and autogenous iliac bone for the treatment ofcervical spondylosis were able to make the neural function improved, culturedegree, occupation, medical cost are the main factors that influencethe operation mode selection;...
