Objective:To study the treatment result and restore intervertebral cervical curvature and height differences of cervical spondylotic myelopathy in anterior approach between interbody autograft iliac bone graft with anterior plate fixation and titanium mesh cage with anterior plate fixation.Methods:According to inclusion criteria and exclusion criteria,33patients (22males and 11 females)with cervical spondylotic myelopathy who underwent anterior cervical decompression fusion operative treatment between April 2008 and October 2009 in Shan Dong university Affilated hospital of triditional chinese medicine were included in this study.There are 18 cases with interbody autograft iliac bone graft and anterior plate fixation and 15 cases with titanium mesh cage and anterior plate fixation.The preoperative,the last follow-up of JOA score, Cobb angle measurement fusion segments of the lordosis (or kyphosis), the value of the cervical lordosis(or kyphosis) D,the height of vertebral fusion segments between HAB and HPB of all patients was evaluated respectively.statistical analytical methods. RESULTS:All patients are given bone fusion after 9 to18months of follow-up (mean 11.3 months). There is a significant difference between preoperative and afteroperative of JOA scores in each group after 3 months and 9 months (p<0.05). Both of the two surgical methods are effective, and there is no difference between the two groups(p>0.05).There is a significant difference between interbody autograft iliac bone graft with anterior plate fixation and titanium mesh cage with anterior plate fixation.of Cobb angle measurement fusion segments of the lordosis (or kyphosis) and the height of vertebral fusion segments of HPB after 3 months and 6 months (p<0.01).Otherwise, There is no difference between the value of the cervical lordosis(or kyphosis) D and the height of HAB in two groups after operation(p>0.05). CONCLUSION:Both the titanium mesh and autograft iliac bone groups have achieved good results in the treatment of cervical spondylotic myelopathy. There is no difference in treatment results of the two groups and to maintain the curvature of cervical spine. But the titanium mesh group in the maintenance of fusion segments vertebral height HPB and lordosis of the Cobb angle is superior to autograft iliac bone group.
