Meta-analysis Of Efficacy And Safety Between Embolization And Surgical Clipping For Anterior Communicating Aneurysms

Objective To analyze the postoperative complications,discussanterior communicating aneurysms before the best surgical approach,provide theoretical basis for further treatment by application ofevidence-based medicine system evaluation method at home and abroadto Hunt class3(or<3) under the anterior communicating aneurysms early(within72hours) before surgical operation clip with interventionalembolization treatment of postoperative efficacy evaluation system.Methods By retrieving Science-Direct, PubMed, China journalfull-text database (CNKI), and other domestic and foreign literaturedatabase, collecting on January1,2000to June30,2014for anteriorcommunicating aneurysms. Survey for aneurysm case-control literature,adding retrieval into the study of references, a careful analysis ofliterature, according to the related standard into and out of literatureinformation, sorting and according to Newcastle-scale on data qualityevaluation in Ottawa, using RevMan5.1software operative method andtherapeutic effect in the references, complications and related results ofMeta analysis.Result:After Systematic analyzing data, a retrospective analysis ofpaper has16, a total of four diagnosis before clear traffic aneurysmpatients into the study, in which all patients with N underwent surgery onembolization therapy, there are N the microsurgery clip treatment. Meta-analysis showed: embolization of microscopic clip after shorterhospitalization time, low incidence of electrolyte disorder (all P <0.05),while in hydrocephalus and vasospasm, bleeding again, recurrence of theaneurysm, the fatality rate of no statistical significance (P>0.05).Conclusions: Endovascular treatment have little damage to the braintissue vice, shorter hospitalization time, quick recovery, low incidence ofelectrolyte disorder advantage. Aneurysm embolization with clip incerebral vasospasm and hydrocephalus and rebleeding morbidity have nodifference, but the relapse rates were higher in the aneurysm. Formeraneurysms interventional embolization treatment recommendationschoice for transportation, but it requires the clinicians have richexperience, exquisite technology and advanced equipment, to achievedense embolism aneurysms, reduce the recurrence rate.
