Objective:Analysis the advantages and clinical value of gasless laparoscopicgynecological surgery.Methods:Collected33gasless laparoscopic surgery patients from February2013to February2015in our hospital and the second hospital (gaslessgroup), and collected35laparoscopic surgery patients in the same period(gas group). Comparative analysis of two groups by operation time,intraoperative blood loss, hospitalization days and total hospitalizationexpenses, postoperative complications, and compare with the ovary sizeand number of follicles, and compare with the preoperative,intraoperative and postoperative respiratory function, blood gasanalysis results.Results:1.There is no significant difference between the number ofleiomyoma that is eliminated during the operation and that is promptedby preoperative ultrasound in gasless group (p>0.05). 2.Respiratory function of gasless group shows no statisticaldifference in pre-operation, intra-operation, post-operation management(p>0.05). Respiratory function of gas group shows statistical differencein pre-operation, intra-operation, post-operation management (p<0.05).3.The results of pre-operative and post-operative blood gas analysisshow no significant differences in gasless group (p>0.05), while the gasgroup shows statistical differences(p<0.05).4.There is significant differences between gas group and gaslessgroup of average operation time, intraoperative blood loss, postoperativecomplications occurrence, average hospitalization days and totalhospitalization expenses (p<0.05). Nevertheless, the catheter removaltime shows no differences between two groups (p>0.05).5.According to the analyzation of ovarian function in the gas group,the size and blood flow of ovary show statistically descending afteroperation(p<0.05).Conclusion:1. Gasless laparoscopic surgery has the advantages of shortoperation time, less bleeding, no change in patients with respiratoryfunction and blood gas analysis results, less postoperative complications and simple, safeļ¼Œreliable and low cost.2. Gasless laparoscopic operation is suitable for gynecologicaldiseases, such as uterine myomas, appendix operation etc. It expands theindications for surgery. Patients with cardiopulmonary dysfunction canalso be offerd this minimally invasive surgical treatment.3. Gasless laparoscopic operation can reduce the postoperativeresidual myoma and also has a protective effect on ovary.
