objective1.To investigate clinical distribution and drug resistance ofAcinetobactor baumannii of the First Affiliated Hospital of guiyang Medical Collage,so as to provide basis for the clinical reasonable use of antibiotics.2.to detectcarbapenemase. Genotype as well as classⅠ and Ⅱintegrase gene for preliminarystudy on the mechanisms of Acinetobacter in our hospital.Methods collected allin-patient and out-patient in the hospital from January2008-December2013bacteriological testing of specimens, through Microscan walkaway40SI automaticbacteria identification instrument for bacterial identification and drug sensitiveanalysis, receive total isolated distinct2649strains of Acinetobacter with its drugsusceptibility data during six years.Hodge test was used for carbapenemasesphenotypic tests on which100resistance drug strains, EDTA collaborative test onwhether produced metal beta-lactam enzyme for phenotypic tests, PCR was used todetect Metal enzyme (IMP,VIM), OXA enzyme (OXA-23,OXA-24,OXA-58,OXA-51),and integrase (Int1,Int2) gene, amplification products by gel electrophoresis analysisand UV imaging. Results The antibiotic resistance rise steadily, until2013they wereall than65%, especially were Imipenem, left-oflOXAcin and ticarcillin/clavulanicacid increasing most significantly. Detection rate of the wound secretion ishighest,Drug resistance of blood sample rate was relatively low; in age groups, mostinfected objects were adult and elderly patients. Adults is highest in the rates ofdrug-resistant. children is lowest; drug resistance in ICU is highest and internalmedicine is minimum.OXA carbapenemases phenotypic test result show61strainswere positive,metal enzyme were all negative. and PCR results showed the number ofOXA-23was100, Int1was100, OXA-51was97, OXA-58was1,and OXA-24, VIM,IMP, Int2was not found. Conclution The drug resistance of Acinetobacter were rise steadily, especially the carbapenemases antibiotics, it warns us should take effectivemeasures to control drug resistance. Acinetobacter baumannii mainly produce OXA-23and OXA-51carbapenemases genes, and all strains carry the classⅠ integrasegene.
