ObjectiveEvaluate the effectiveness and safety of clinical studies on Traditional Chinese Medicine treating Acute upper respiratory tract infection based on syndrome differentiation. Design and optimize the intervention programs, in order to offer some valuable findings to the clinical pathways of Acute respiratory via this study. This research focus on avoiding the abuse of antibiotics and also the standardized use of Chinese patent Medicine under syndrome differentiation. This research could also enhance the clinical effect and reduce the financial burden on the patients. Finally it promotes the clinical data for the popularization and application of the clinical pathway.Design methods, Subjects and Intervention MeasuresAccording to the randomized controlled trail principle, this study has collected patients with upper respiratory meeting the criteria from respiratory medical ward in Hospital of Chengdu university of TCM. Based on the sample calculation formula:N=[8pc(1pc)]/(pl-p2)2and previous study has showed that the effective rate was100%, as well it’s reported that Western medicine’s effect rate97%, we counted the sample capacity was131. Counting in the10%depigmentation, the capacity was144, and expanded to160cases when considering the hierarchical factor. Number the selected patients according to the doctor order, randomly divide them into control group and experimental group according to the proportion of1:1by DPS statistical software. For the controlled group(Compound Dextromethorphan Hydrobromide Tablets),the patients take the Compound Dextromethorphan Hydrobromide Tablets1#Po q12h treatment for five days. At the same time, for the experimental group, on the base of syndrome differentiation, patients of wind-cold type were given Chinese Medicine decoctum of MaJing mixture1agent daily for5days, four times per agent. Patients of wind-hot type were given Chinese Medicine of XiaSangJu particles taken with boiled water for5days, a bag each time and4times a day. Patients of cold-wet type were given Chinese Medicine decoctum HouPuXiaLing decoction,1agent daily for5days, four times per agent. Patients of wet-hot type were given Chinese Medicine decoctum Clearing heat and dissipating dampness oral liquid for5days,20ml each time,3times a day. Both groups are under the same treatment course. By comparison and analysis the TCM symptoms before and after the treatment of clinical symptoms, signs, temperature recovery, objectively evaluate the clinical curative effect of Traditional Chinese Medicine on acute upper respiratory tract infection on the basis of TCM syndrome differentiation.Results(1) In the aspect of TCM symptom score before and after treatment, through t-test analysis, compare total TCM symptom score of the four types of cold, two groups, result showed statistically significant difference (P0.05). It means the treatment is valid in the improvement of TCM symptom score.(2) In the aspect of TCM symptom score between the2groups before and after treatment, through t-test analysis, there’s no statistically significant difference (P>0.05) between the2groups of wind-cold types before treatment, thus it’s comparable. Meanwhile, there’s no statistically significant difference (P>0.05) among the TCM symptom score before treatment. The result indicates that on the improvement of TCM symptom score of Wind-cold type, the controlled group and experienced group are equal. And for the other3types of cold, there’s no statistically significant difference (P>0.05), and it’s comparable. There’s statistically significant difference (P>0.05) among the TCM symptom score after treatment. It indicates that in the improvement of TCM symptom score of cold of wind-hot type, wet-hot type and cold-wet type, the experienced group is better than the controlled group.(3) In the aspect of the total curative effect, we apply rank sum test to evaluate the effect of the2kinds of treatment on wind-cold type cold. As examined, P>0.05, there was no statistically significant difference, which means the2treatments on the wind-cold type had equal effect. Also, rank sum test examined the other3types of cold, P<0.05, there was statistically significant difference, which means the experienced group was better than the controlled group.(4) In the aspect of temperature recovery rate after treatment, after X2test analysis, two methods to treat the4kinds of cold has no statistically significant difference (P>0.05) in the temperature recovery cases and temperature recovery rate, which indicates that both the experienced group and controlled group are equal in the temperature recovery condition.Conclusion:Treat acute upper respiratory tract infection based on TCM syndrome differentiation can significantly reduce TCM symptoms, clinical symptoms, signs and increase temperature recovery rate. Also it can avoid the phenomenon of antibiotics abuse and contribute to the standardized use of Chinese patent Medicine under syndrome differentiation. This could also enhance clinical effect and reduce the financial burden on the patients. Finally it promotes the clinical data for the popularization and application of the clinical pathway.
