| Objective:Evaluation of application effect of endoscopic biopsy iodine staining technique in esophageal screening in Xianju County from2010to2014.Methods:From2010September to July2014, we chose people from the Baita Town and Tian Town in XIANJU, high-risk groups for40-69year-old adopted esophageal screening, while the cardia mucosa were observed suspicious lesions multiple biopsies, and after pathological diagnosis. Esophageal mild dysplasia followed up at intervals of3years, moderate dysplasia followed up once a year.ResultsWe dectected569cases of esophageal dysplasia and esophageal cancer in the8081 cases, the detection rate was7.04%, which esophagus mild dysplasia, moderate dysplasia, severe dysplasia/carcinoma in situ, mucosal and invasive cancer detection rate was5.36%(433cases), respectively,1.19%(96cases),0.42%(34cases),0.05%(4cases),0.02%(2cases), esophageal cancer early diagnosis rate (esophagus severe dysplasia/carcinoma in situ+early cancer/dysplasia more severe esophageal lesions) of95%(38/40). Detection of dysplasia and gastric cardia gastric cardia total of65cases, the detection rate was0.8%. Where the cardia low grade intraepithelial neoplasia, high-grade intraepithelial neoplasia and early cancer, invasive cancer detection rate was0.46%(37cases), respectively,0.22%(18cases),0.12%(10cases). Cardia early diagnosis rate (within the high-grade intraepithelial neoplasia and early cancer/high-grade intraepithelial neoplasia and early invasive carcinoma+) amounted to64.29%(18/28). We had completed the follow-up of219cases esophagus in patients with mild to moderate atypical hyperplasia, including182cases of mild dysplasia, moderate dysplasia in37cases,78.3%(133cases) of patients with mild dysplasia in2to3years after complete remission of dysplasia,21.98%(40cases) of patients remained unchanged,4.95%(9cases) of patients can be aggravated into moderate dysplasia, severe dysplasia or no change over the lesions.48.65%(27cases) of patients with moderate dysplasia fully subsided or into a mild one to two years,18.92%(7cases) of the patients remained unchanged,8.11%(3cases) transformed into severe dysplasia or cancer.Conclusion:Esophageal screening and follow-up could increase the detective rate of esophageal dysplasia and esophageal cancer, facilitating early treatment. |
