BackgroundPars interarticularis defects in lumbar vertebra is a common clinical disease, its morbidity is about5%in adults. It is an important reason for low back pain and other symptoms. Additionally, it may result in lumbar spondylolisthesis. Thus, early clear diagnosis of lumbar spondylolysis and differentiation between types can avoid the lumbar spine instability or lumbar spondylolisthesis, and raise the quality of life. The lumbar traditional examinations are comprised with double oblique X-ray plain film and CT scan. Now the digital tomosynthesis has become a new method for diagnosis of lumbar interarticularis defects. It has a higher specificity, sensitivity and accuracy. There are very few studies about the use of DTS in lumbar spondylolysis. This study is a retrospective analysis on the DTS in lumbar spondylolysis. The purpose is to investigate the application value of DTS, and to provide the imaging basis for clinical treatment.PurposeTo investigate the application value of digital tomosynthesis (DTS) in diagnosis for simplex pars interarticularis defects in lumbar vertebra.Methods39patients who were suspected as pars interarticularis defects were underwent DTS examination. Then it was determined that if the pars interarticularis defects existed, and whether one side or two, et al. In addition, the sensitivity and coincidence were analyzed. ResultsIn radiograph, pars interarticularis defects were diagnosed clearly in19patients,8cases were uncertainty and12patients were eliminated. After the DTS examination, pars interarticularis defects was diagnosed clearly in32patients,2cases were uncertainty and5patients were eliminated. The sensitivity and accuracy was55.9%and61.5%respectively in radiograph, and was94.1%and94.9%in DTS. There were significant differences between the coincidence in DTS and in radiograph (x2=11.08, P<0.05)ConclusionDTS can display the pars interarticularis defects clearly and the types of it (eg. one side or two, et al). Comparing with radiograph, DTS can markedly increase the detection rate. It can be an effective supplementary for radiograph.
