Objective:To analyze the factors affecting operative blood loss from open radical hysterectomy and pelvic lymphadenectomy±paraaortic lymph node sampling surgery for cervical cancer. Investigate the approach to reduce the operation blood loss. Methods:Clinical, pathological, and operative data of193women with cervical cancer stage ⅠA2-ⅡA and ⅡB suffered neoadjuvant chemotherapy had open radical hysterectomy with bilateral^pelvic lymphadenectomy (RHPL)±paraaortic lymph node sampling surgery at the First Affiliated Hospital of Guangxi Medical University from January2011to December2013were reviewed retrospectively. Results:the mean blood loss was300ml (50-3000ml). The mean operative time was260±62min. long operation time was statistically associated with operative blood loss of300ml or more from multivariate analysis. The more number of lymph nodes, long operation time were statistically associated with operative blood loss of800ml or more. Early clinical stage may lead to less bleeding, but there were confounding factors. Conclusions:the more dissection of the lymph nodes and the longer operation time, the more risk of operative blood loss. To reduce operation blood loss,we should pay attention to reduce operative time,which need to improve surgical technique, at the same time, introduce advanced surgical equipment and surgical method. Early clinical stage may lead to less bleeding. preoperative anemia, obesity, age over50years, previous surgery and preoperative chemotherapy may not increase the risk of bleeding with careful and standardized operation.
