Purpose:To study the clinical manifestations and the diagnosis and treatment of subarachnoid neurocysticercosis in order to improve the knowledge of this disease among clinician. Methods:The clinical data of7case with subarachnoid heurocysticercosis dignosised between2003and2013at the Guangxi medical university.Relevant literatures-40cases with subarachnoid neurocysticercosis are reviewed.Resul:All the seven cases are males with the age ranging from thirty-two to sixty-three and the median is fourty-flve. The course of the disease is three years (two weeks, ten years) Four of the cases had a epidemiologicalhistory of taenia solium.Six with a sign of high intracranial pressure.Three patients appeared vision changes.Two had a motor system involvement.Two had a epileptic seizure.One appeared a senior nerve dysfunction.None of cases had a positive of meningeal irritation sign. Serology:7/7had a Positive reaction of cysticercosis enzyme mark.3/7had a increasedeosinophilic granulocyte. Stool culture:one case discoveredegg of taenia solium in stool.CSF:intracranial pressure change(4had a high intracranial pressure and3had a low one).3/7had a slight change in glucose and chloride.Protein change(6/7had a qualitativeabnormal change and3/7had a quantitativeabnormal change).3/7had a multicellular appearance.all the four cases who had a test of ADA are normal.All the seven patients had a positive reaction of cysticercosis enzyme mark.7cases had a abnormal manifestation of Neurocysticercosis in cranial MRI.3of6patients who had a enhance scanning are enhancing lesions.Only one had a histologic demonstration during the surgery.CONCLUSION:1.Neurocysticercosis is a common parasite sited in CNS and the involvement of subarachnoid space is rare.2.The clinical manifestations of subarachnoid neurocysticercosis depends on the location and the size of cyst, intracranial hypertension is common appearance and often associated with symptoms compatible with cranial nerve involvement and "stroke" like onset.There is a various clinic manifestation and with the help of immunology,neuroimaging,pathological biopsy as well as epidemiology,the diagnosis can be easier.3. The subarachnoid neurocysticercosis had a bad reaction to Anthelmintics but with good outcome throuth neuroendoscopic excision,which can be a optimization.
