ObjectiveCervical pedicle screw provides a great stabilization effect by fixing front,middle andposterior column of cervical Spondylosis at the same time. Biomechanical studies revealedthe superior stabilizing effect of pedicle screw fixation compared with other internalfixation procedures used with the cervical spine, including lateral mass screw fixation.Since its lower loose rate at the bone-screw interface and greater pullout strength,cervicalpedicle screw technology is widely used in clinical practice. In despite of its advantages inbiology, complications such as vascular nerve injury should also be paid much attention.This study is to evaluate the adult lower cervical pedicles by using CT scan,measurethe relative parameters to get reliable relative data and explore selection principle ofdirection of cervical pedicle screw and its clinical meaning.Clinical Materials and Methods1. General Information The study consisted of60patients who were underwentCT scanning of cervical vertebra during Oct2012to Oct2013in Affiliated Hospital ofTaishan Medical College. Those who had cervical spondylopathy and osteopenia shown onCT were excluded. Detailed record general clinical materials such as gender, age andheight.2. Equipment Callipers(Accurate to0.01mm),digital camera(FinePix Z91),spiralCT scanner,CT measurement equipment3. Methods CT scanner system was used. C3-C7vertebrae were scannedcontinuously with a thickness of0.625mm. From sagittal section images of the pedicles,the author cut paralleled the pedicle axis with a thickness of0.4mm to obtain the transverseview which paralleled the pedicle axis. Thus in the transverse view, the reformatted CTcuts were made vertical to the pedicle axis and get the coronal section. All the parametersof the smallest side pedicle section were chosen to measure. 4. Statistical Analysis SPSS11.0software was performed for all statisticalanalysis. Data were described as Means±standard deviations (x s).Statistical differencesof the parameters were compared between right and left pedicles and between male andfemale patients at every level using the indepent t-test.Results1.LA、LB、PD in the C7is the smallest, LA、LB in the C5is the longest,PD inthe C3is the longest. Compared between men and women had significantly different(P<0.05).2.PH is greater than PW in the same segment, PSH is greater than PSW in the samesegment,and PW, PSW in the C3-C7segment increased gradually.3.TA、SA in the C7is minimumin, TA in the C4is maximum, SA in the C3ismaximum, both in men and women without significant difference (P>0.05), the othersection of men and women are significantly different (P<0.05).Conclusion1. Our study indicates that CT scan of cervical pedicle is necessary before operation.The placement of cervical pedicle screw should be individualized and based on detailedpreoperative planning.2. Our study provides reference for the decision-making of selection, direction andlength of screw during cervical pedicle internal fixation operation.
