Objective:To establish clinical case-based diarrhea surveillance network, optimize a set of standard and operational technical protocol for multi-pathogen bacterial diarrhea test in the field. Generally learn the prevalence, composition and change rule of common intestinal bacteria. Build up PFGE database of common intestinal bacteria and to explore the new surveillance model for bacteria diarrhea for PFGE subtyping at local level. Methods:Hongta district of Yuxi city is selected as the surveillance site for laboratory-confirmed diarrhea disease surveillance established from June to December of2012. Collect basic information of diarrhea cases and stool specimen, with traditional method for the isolation and initial identification of diarrhea genie Escherichia coli, Salmonella, Shigella, Aero Monas, Plesiomonas and Pathogenic Vibrio. Strains are double checked with the method of biochemical system or fluorescent PCR. PFGE subtyping for positive strains was conducted for similarity analysis with software BioNumerics5.01.Results:Since June2012, surveillance network is officially established and run very well; multi-pathogen technical protocol is optimized in practice.703stool specimen of diarrhea cases have been collected from June to December, A total of692eligible specimen were tested,155target bacteria have been detected, detection rate is22.4%. diarrheagenic Escherichia coli of positive samples accounts for37.4%, Aeromonas(29%),Shigella(12.3%), Salmonella(11%), Plesiomonass-higelloides(9%), Vibrio(1.3%). diarrheagenic Escherichia coli were divided into53different banding patterns, Aero monas (48), Salmonella (13) and Shigella (12). diarrhea genie Escherichia coli, Aeromonas,Plesiomonas and salmonella have the same strain of banding pattern. Conclusion:From June to December,2012, total positive rate of six target bacteria among diarrhea patients in Hongta district ranges from15.8to31.8%, positive rate of autumn and winter is lower than summer. Diarrhea genie Escherichia coli and Aeromonas are the major pathogens in local. Shigella, Salmonella and Plesiomonas shigelloides also account for certain amount. Diarrhea caused by local Pathogenic Vibrio is rare. Although no bacteria diarrhea outbreak was detected from epidemiologic surveillance in that period of time, diarrhea case infected with the same clone strains indeed existed. Therefore, with continuous improvement of laboratory-confirmed bacterial diarrhea cases extend the technology of bacteria PFGE or other serotyping method at local level will assist to improve the capacity of early detection of outbreak of bacterial diarrhea and shared exposure source.
