Objective:Compared with modified Judet incision and scapular posterior approach inthe treatment of scapular neck fracture of shoulder joint function and complications,provide for clinical selection of therapeutic methods.Methods: In this study, eases from the Affiliated Hospital of Shandong Universityof Traditional Chinese Medicine orthopedic ward, emergency and outpatientservices. From2011September to2013October in Shandong Provincial TraditionalChinese Medical Hospital from31cases of patients with scapular nock fractureoperation,17cases with modified Judet incision operation,14cases with scapularposterior approach operation, two cases were combined with locking reconstructionplate for operation treatment, and two cases of intraoperative and postoperativefunction of the shoulder joint and complications were analyzed.Result: All patients were followed up for.1. The modified Judet incision operation group: the patients were followed upfor6months to24months after operation, the mean fo】low-up time was16.5months,17patients showed no plate fracture, loosening of internal fixation, no woundinfection, necrosis, satisfactory clinical bone healing in9to14weeks. Blade length of12±1,5cm,75±18minutes of operation time, intraoperative bleeding volume wasl43±36ml, postoperative drainage wasl57±29ml, postoperative drainage, postoperative according to Hardegger scoring system, there were excellent in11cases, good in5cases,1cases, the excellent and good rate was94.12%.2. The scapula posterior approach operation group:the patients were followed up for6months to24months after operation, the mean follow-up time was17.3months,14patients showed no plate fracture, loosening of internal fixation, no wound infection, necrosis,9to14weeks to get clinical bone healing. Blade length of15±1.7cm,92±19minutes of operation time, intraoperative bleeding volume was168±43ml, postoperative drainage was183±33ml, postoperative according to Hardegger scoring system, there were excellent in8cases, good in4cases,2cases, the excellent and good rate was85.71%.Conclusion:Modified Judet incision and scapular posterior approach in treatment of scapular neck fracture with short incision operation, short operation time, amount of bleeding and postoperative drainage volume less fracture, combined with Hardegger shoulder score improved operation group, the excellent and good rate was significantly higher than that in the posterior approach group, two groups of indexes had significant differences (P<0.05). With little trauma, short operation time, less bleeding fractures using a modified Judet incision in the treatment of scapular neck, has the advantages of good postoperative recovery.
