| Objective: Posterior circulation ischemia is a common ischemic cerebralvascular disease, accounting for about20~25ischemic stroke. PCI once thatoccurs, the high risk of poor prognosis.Etiolgical diagnosis of PCI more andmore attention in recent years, previous studies have focused on external factorsrather than vascular factors in the blood vessel such as artery variation itself,especially research on the variation of vertebral-basilar artery system and PCIhave seldom reported.To investigate the prevalence of vertebral-basilar arterysystem variation and its imaging characters in patients receiving cerebral arterydigital subtraction angiography.Methods:We reviewed1240patients receiving cerebral artery digitalsubtraction angiography, and got the prevalence of vertebral-basilar arteryvariation and associated cerebral artery disease in the patients.Results:Among1240cases in patients with ischemic cerebral vascular disease,vertebral artery origin variation were detected in26cases (2.09%), of which20males,6females with an average age of55.2. Total of328cases of vertebralartery dysplasia (26.5%),256males and72females, which detected unilateralvertebral artery hypoplasia of314cases, the combined circulation ischemia of206cases, incidence of vertebral artery hypoplasia in58cases, normaldevelopment of vertebral artery lateral incidence of148cases, Normaldevelopment of vertebral artery of posterior circulation ischemia occurs abovethe artery hypoplasia, a statistically significant difference (P<0.05);293cases ofnormal lateral stenosis of vertebral artery, which70%the incidence of vertebralartery stenosis greater than normal side142cases,130cases of posteriorcirculation ischemia stenosis is less than70%151cases,18cases of posterior circulation ischemia and normal lateral stenosis of vertebral artery and posteriorcirculation ischemia there are differences, the difference was statisticallysignificant (P<0.05).the fenestration of the vertebral-basilar artery were5cases,.among the fenestrations of cerebral artery,1case localed in the beginningof vertebral artery,1case in vertebral artery V3,5cases of fenestrations bloodsupply of vertebro-basilar artery showed no consistent symptoms or signs. wefound4cases primitive trigeminal artery, all belonged Salztman â… type, andshowed no consistent with arterial ischemic stroke. Variation of vertebro-Basilar system in the pathogenesis ofposterior circulation ischemia plays a very important role, when one side ishypoplastic vertebral artery, lateral position of vertebral artery lesions mayeasily lead to posterior circulation cerebral infarction;2. Cognition and mastery of the vertebral-DSA imaging and hemodynamiccharacteristics of the basilar artery system variations, has important clinicalimplications for the diagnosis of cerebrovascular disease, and the developmentof interventional treatment programs. |
