Objective:Discuss with bilateral joints under the ground lock the necessity and feasibility of anterior cervical spine fracture dislocation operation.Methods:1.Through review of literature at home and abroad and our previous research work, design and put forward a way of anterior unlock reset operation.2.Screening of2010-2014merger bilateral joints under the ground lock the18cases of cervical spine fracture dislocation. The Ⅱ°4cases,8Ⅲ, Ⅳ°in5cases, completely dislocation in1case. Anterior surgery and the surgical approach to unlock reset, according to the patients in some cases with posterior surgery, the patient of spinal cord and nerve function with Frankel classification records and evaluation.Results:Design and puts forward the anterior unlock reset operation method is reasonable, and is feasible in theory.2.13cases of incidence completely reset, reset reached more than90%in5cases.4cases of incomplete spinal cord injury, nerve function for different degrees of improvement, Frankel score average increase12level. No cases of spinal cord and nerve function is aggravating, of which1case was completely dislocation without spinal cord injury, only the treatment of bilateral cases of nerve root irritation process suggests the necessity of anterior surgery.Conclusion:1.Before cervical dislocation with bilateral joints in anterior surgery is feasible, is necessary.2.Anterior operation method is the most fundamental and most major surgery, posterior surgical procedure is secondary and auxiliary operation method.3.Posterior surgical procedure in the patient’s general condition permits it to indications are as follows:①Vertebral posterior structure was badly damaged, and compression the spinal cord and nerve root.②Unlock the reset by anterior cervical surgery, but behind the articular process did not reach anatomical reduction, and there is buckling instability.③The patient compliance is poor, or existing early hypostatic pneumonia for sitting up easy care, need strong fixation.
