Objective: By investigating the constitutional ratio, the epidemiologyand pathological characteristics of primary tumors in parotid gland andexploring the pathogenic characteristics of multiple primary tumors inparotid gland in Jilin province, it is helpful to provide directions for theclinical diagnosis and treatment of parotid gland tumor.Material and method: All the clinical data of623cases with primarytumors in parotid gland were collected from January2011to January2015in the department of oral and maxillofacial surgery in Jilin University. Thegender, age, attack time, regional position and pathological type of all623cases were separately analyzed by scientific statistics. Finally, we furtherexplored the connection among pathological type, attack time, regionalposition and living habits on38cases of primary tumors in parotid gland.Result:1. Composing ratio of patient resources: the rural patients take higherproportion than urban patients, about398cases, which comprise about63.8percent of all collected patients; And there are225cases of urban patientsthat occupied over36.12percent.2. Composing ratio of benign and malignant tumors, sex and age:534cases were benign tumors that took up86%.89cases were malignant thattook up14%of all cases. There are6benign cases for every1malignant,334were males and289were females, allocating just54%and46% respectively. The sex ratio of males and females is1.16:1, from13to82years of age, and it strikes most often at the ages from51to60years(29.86%), followed by41to50years (22.95%).3. Composing ratio of unilateral primary tumors and multiple primarytumors in parotid gland, and tendency in right or left: there were585casesof unilateral primary tumors in parotid gland that took up93.9%,38caseswere multiple primary tumors that occupied about6.10%. The patients ofmultiple primary tumors that prefer to take place on left-side were8cases(21.05%), and patients who prefer to take place on right-side were8cases(21.05%). The rest of all cases were22(57.89%) with patients who hadbilateral tumors, and the ration among them were0.36:0.36:1, the ratio ofmultiple attack between homolateral and bilateral was0.72:1, and theformer had the higher incidence.4. Composing ratio of pathological type of unilateral primary tumorsin parotid gland: In benign tumors, pleomorphic adenoma was the mostcommon one that altogether255cases, with the frequency of47.75%; Andsecondly is Warthin’s tumor, altogether167cases, with the frequency of31.27%; The next one is basal cell adenoma,60cases, with the frequencyof11.24%. In malignant tumors, there are18cases of mucoepidermoidcarcinoma which took highest proportion of all, about20.22%; Andsecondly is cancerous pleomorphic adenoma, altogether11cases, with thefrequency of12.36%; Followed by9cases of cystadenocarcinoma that account for10.11%, and9cases of non-Hodgkin’s lymphoma that accountfor10.11%.5. The characteristic of multiple primary tumor(MPT) of parotid gland:there are38cases of MPT of parotid gland, accounting for6.10%of allcases. As for pathological types, the most common one is bilateralWarthin’s tumor for19cases, accounting for50.00%of the MPT cases,Followed by multiple unilateral Warthin’s tumor for12cases, accountingfor31.58%. There are2cases of bilateral basal cell adenoma whichaccounting for5.26%,1case of multiple unilateral pleomorphic adenomaswhich accounting for2.63%,2cases of Warthin’s tumour plus pleomorphicadenoma,1case of unilateral Basal cell adenoma plus pleomorphicadenoma, and1case of unilateral eosinophilic adenoma plus pleomorphicadenoma. For all MPT of parotid gland case,27were males and11werefemales, the ratio was2.45:1, from26to80years of age, average age is58.6. The characteristic of pleomorphic adenoma(PA): Among260casesof PA, there are255cases of monofacal PA and5multifocal PA or PA plusother type of primary tumors, accounting for41.73%of all cases and48.69%of benign tumors. Among which, there are102males and158females and the ratio between male and female is0.65:1. The age range ofonset of the PA cases is18~71years. It’s most likely occurred in patientsbetween age41and50, accounting for27.31%.7. The characteristic of Warthin’s tumor(WT): Among200cases of WT, there are167cases of monofacal WT and5multifocal WT or WT plusother type of primary tumors, accounting for32.10%of all cases and37.45%of benign tumors. Among which, there are162males and38females and the ratio between male and female is4.26:1. The age range ofonset of the WT cases is31~80years. It’s most likely occurred in patientsbetween age51and60. Among which, there are181smokers(90.50%) and19non-smokers(9.50%), the ratio is9.54:1.8. The characteristic of Basal Cell Adenoma(BCA): Among63casesof BCA, there are60cases of monofocal Basal Cell Adenoma and3casesof multifocal BCA or BCA plus other type of primary tumors, accountingfor10.11%of all cases and11.80%of benign tumors. Among which, thereare19males and44females and the ratio between male and female is0.30:1. The age range of onset of the BCA cases is26~74years. It’s mostlikely occurred in patients between age51and60.Conclusions:1. In the local area, the rural patients take higher proportion thanurban patients, and benign tumors are more common than malignant tumors.The tumors are more likely occurs in elderly men.2. The most common monofocal primary benign tumor is PA,followed by WT, then BCA. The most common primary malignant tumor ismucoepidermoid carcinoma, followed by cancerous pleomorphic adenoma,then cystadenocarcinoma and non-Hodgkin’s lymphoma. 3. In this study, all the multiple primary tumors of parotid gland arebenign, altogether of38cases, accounting for6.10%. The most commonpathological type is bilateral Warthin’s tumor, followed by multipleunilateral Warthin’s tumor, then PA plus other type of primary tumors.
