Background and objective: Many researchers found that the different kinds oflipids and lipoproteins were closely related to the presence and development ofcerebral atherosclerosis.However, the relationship between lipids and cerebralatherosclerosis has not been fully elucidated.It still needs to be further studied. Thisresearch aims at the analysis of the relationship between lipids and cerebralatherosclerosis, providing more evidence for the prevention and treatment of cerebralatherosclerosis.Methods: Selected patients who had taken TCD and CA examination fromneuro department in the1stHospital of Jilin University from October2013toOctober2014as objects, excluding the patients who had arterial dissection,vasculitis, cerebral vascular malformations, the potential cardiogenic embolism andwhose data of.blood lipids and TCD, CA was incomplete. Clinical data wasrecorded, including age, sex, the history of hypertension, diabetes mellitus, smoking,drinking and laboratory information which examined in24hours,including bloodpressure, blood lipids, fasting glucose, hypersensity C reactive protein (hs-CRP),folic acid, vitamin B12, uric acid, homocysteine and so on. Then devided the patientsinto different groups. There were two ways:(1)According to the severity of theatherosclerosis: normal group(no obvious abnormalities in TCD and CA),thickeninggroup (only visible carotid internal artery wall thickening in CA), plaque group(visible presence of carotid plaques in CA, no stenosis in TCD and CA), mildstenosis (the rate of stenosis<50%), moderate stenosis (the rate of stenosis is50%to69%), severe stenosis (the rate of stenosis is70%to99%) or occlusion group(could’t explore the blood flow signal of the related artery).(2)According to thevascular stenosis: stenosis group and non-stenosis group. At last,the information of patients was statistically analyzed to explore the relationship between lipids andcerebral atherosclerosis.Results:(1) In this study, a total of3381cases was studied,there was356cases (10.5%)in normal group,89cases (3%)in thickening group,1199cases (35.5%) in plaquegroup,409cases (12.1%) in mild stenosis group,134cases (4%) in moderatestenosis group,1194cases (35.3%) in severe stenosis or occlusion group..(2) According to the AS severity: with the severity of cerebral AS increasing,the mean of HDL-C levels were1.21mmol/L,1.20mmol/L,1.17mmol/L,1.15mmol/L,1.12mmol/L,1.11mmol/L,respectively(P <0.05). It couldn’t be seen fromthe research that the obvious trends of TC, LDL-C, apo A1, apo B, Lp (a),non-HDL-C, LDL/HDL, AIP existed in different degree of atherosclerosis. But theirmean were highest(except for apo A1) or lowest(apo A1) in different stenosis groups(P <0.05). The relationship between TG and cerebral atherosclerosis was notstatistically significant (P=0.577>0.05).(3)According to the stenosis of cerebral artery:after univariate analysis, thefactors associated with cerebral artery stenosis included age, HDL-C, LDL-C, apoA1, apo B, Lp (a), non-HDL-C, LDL/HDL, AIP, Hcy, GLU, HbA1c, hs-CRP,FA,SBP, PP, sex, hypertension, diabetes history (P <0.05). After binary logisticregression analysis, the independent risk factors of cerebral artery stenosis in turnwere male(β=0.63,OR=1.88,P=0.000), low HDL-C(β=-0.45,OR=1.88,P=0.000),high non-HDL-C(β=0.11,OR=1.12,P=0.022),high fasting blood glucose (β=0.10,OR=1.11,P=0.000) and high hs-CRP(β=0.01,OR=1.01,P=0.002).(4) The correlation coefficient of LDL and apo B was0.859(P=0.000).Thecorrelation coefficient of HDL-C and apo A1was0.697(P=0.000).Conclusions:(1) With the cerebral atherosclerosis severity increasing, HDL-C level isdeclining..(2) The lipids associated with cerebral artery stenosis include HDL-C, LDL-C,apo A1, apo B, Lp (a), non-HDL-C, LDL/HDL, AIP (P <0.05).After the exclusion of relevant confounding factors,the independent risk lipids factors of cerebral arterystenosis were low HDL-C and high non-HDL-C(P <0.01).(3) In addition,LDL-C and apo B, HDL-C and apo A1have a certain relevance.
