Objective:To evaluate the outcome of spleen-preserving distal pancreatectomy and explore its feasibility. To compare spleen-preserving distal pancreatectomy and distal pancreatectomy and splenectomy and explore their advantages, disadvantages, indications and contraindications, to find a reasonable way of surgical treatment for tumour of distal pancreatectomy.Methods:The clinical data of11cases of spleen-preserving distal pancreatectomy (SPDP group) and8cases of distal pancreatectomy and splenectomy(DP group) from November2011to January2014in the Qilu Hospital of Shandong University were analyzed retrospectively.Results:Among11cases of SPDP group,4cases of cystadenoma,3cases of insulinoma and lease of solid-pseudopapillary tumors of pancreas,1case of Intraductal papilloma,l case of Pancreatic ductal adenocarcinoma,and1case of Neuroendocrine carcinoma were included.There were no postoperative deaths, pancreatic fistula occurred in4patients, and was cured within3-6weeks by non-operative treatment.Among8cases of DP group,1cases of cystadenoma, lease of solid-pseudopapillary tumors of pancreas,3case of Pancreatic ductal adenocarcinoma,1case of Neuroendocrine carcinoma,1case of Pancreatic cystadenocarcinoma,and1case of splenic arteriovenous fistula were included.There were no postoperative deaths, pancreatic fistula occurred in2patients, and was cured within4weeks by non-operative treatment.Conclusion:The intraoperative bleeding volume, postoperative complications and hospitalization time of spleen-preserving have no significant difference with traditional distal pancreastectomy. but the platelet maximum value postoperative is lower, it is a safe, feasible and effective mode of operation, should become the preferred treatment for the patient of the benign tumor of body and tail of pancreas and early invasion of pancreatic carcinoma of body and tail.
