Background:After the large comprehensive hospital or cancer specialist hospital treatment, cancer patient required continuous review and subsequent rehabilitation treatment, and community health organizations play an important role in this process. Clinical information of cancer patients were not well saved currently, in order to save cancer patient’s clinical information, Department of Oncology in the First Affiliated Hospital of Guangxi Medical University established a system about saving information for cancer patients:Department of Oncology in the First Affiliated Hospital of Guangxi Medical University-Community Medical Tracking Management System.Purpose:Discussed the management method of hospital-community medical tracking system which based on the network, Understand the situation and the shortcoming of the system, analyzed and explored the solution. The aim was improved the diagnosis and treatment, improved quality of life and prolong survival for cancer patients.Methods:Collected treatment related information of community cancer patients, which went to the First Affiliated Hospital of Guangxi Medical University Medical Oncology or local community health agency or around the cities and counties hospital. It included clinical and general information. General information:name, gender, age, treatment units, nationality, occupation, family history, contact information. Clinical information:clinical data, onset date, disease names, staging, chemotherapy time, radiotherapy time, operation time, treatment time, PS score, interventions. Then put all the information into the System, and formulated relevant management method in all the aspects, then telephone followed up the patient from January2013to December2013. Analyzed the information and follow-up results included in the system.Results:It was successfully collected250cases information. Successed to manage the system with informatization. It contained132male cases (52.8%),118female cases(47.2%), collected47cases of colorectal cancer (18.8%), breast cancer46cases (18.4%), liver cancer13cases (5.2%), gastric cancer21cases (8.4%), lung cancer29cases (11.6%), non-hodgkin’s lymphoma16cases (6.4%), nasopharyngeal carcinoma10patients (4%), others68cases (27.2%). Logined the system through network, used the system search methods to search patient’s information, we cound retrieve information fully、accurately and quickly. From january2013to December2013,107cases were telephone followed-up included in the system,25lost to follow,17deaths,65survival,86%of the survival could come back to chemotherapy on time,95%had good medication compliance,91%could review regularly.It worked well since2008when the system was built. However,there were some shortcoming for the system. Such as, it affected the normal use of the system with computer failure and crowded network, some community doctors was not unskilled operation the system, community doctors less frequently used system to input clinical information, recorded medical information was too simple. Conclusion:1. It formed a method to manage the hospital-community tracking medical system through network about community cancer patients. Successed to share cancer patients’clinic information between a large comprehensive hospital and community, provided a reference for community management.2.It had great significance of the hospital-community medical tracking system in theory and practical clinical work, so it could further promote to use. In the subsequent work, It is necessary to set up case-control study to prove that whether the system improve the patient’s quality of life and survival time.3. There were some short-coming for the system. In order to make the system run better,it needed to raise awareness in all the staff on medical systems, and increased financial input to transferred patients to community medical institutions for treatment and attracted more professionals to the community, improved the knowledge about prevention and treatment on cancer for community physicians, further improved the system’s function.
