Surgical Treatments For Thoracic Disc Herniation

Objective:To evaluate the results between the two surgical ways that is transpedicular approach and transthoracic approach in the patients with TDH.Methods:The clinical date of42patients (aged from24years old to72years old, mean age is49years old)with TDH, from July2004to December2013,were retrospectively analyzed. There were31male and11female among these patients.20cases were treated by transpedicular approach and the other22cases followed transthoracic approach.JOA and ASIA score were calculated before and after operations, and operative time and lost blood were counted. Patients were followed2-74months. All the dates were analyzed by statistics.Results:All patients were better than operations. Average dates came from those patients who were treated by transpedicular approach surgical way:blood loss348.00ml,operative time133.75minutes, inpatient13.05days, pre-operation JOA scores4.65and post-operation JOA scores7.95,recover rate52.09%,pre-operation ASIA grade C12person, D8person, post-operation ASIA gradeB2person, C6person, D9person, E3person, last follow up ASIA grade C4person, D12person, E4person, on the other way,on transthoracic approach way patients produced the dates:blood loss386.82ml,operative time143.64minutes, inpatient14.26days, pre-operation JOA scores4.73and post-operation JOA scores8.45,recover rate59.94%,pre-operation ASIA grade C14person, D8person, post-operation ASIA gradeB1person, C5person,D12person, E4person,last follow up ASIA grade C2person, D15person, E5person. Some of all patients appeared complications after operations and were treated actively. All patients were followed up38months averagely, and felt satisfied. Conclusions:Both transpedicular approach and transthoracic approach were the good way to treat TDH,but transpedicular approach lost blood less and save little time,as well as transthoracic approach had higher recover rate than the other one.
