objective study the exposure of the high transthoracic approach for anterior decompression and instrumentation at the upper thoracic vertebrae. Methods There were there patients, two male and one female, The lesion involed T3 to T5 vertebrae,Among of them,there were two tuberculosis including T3\4 and T4\5, another was fracture of T3\4 Vertebrae.All patients operation throngh high transthoracic approach,we treat tuberculosis through lesion clearance, bone graft and internal fixation were performed for vertebrae fracture. Rseults successful exposure lesion ,clearance decompression and implant were made in all patients ,there were no important Vascular and nerve injury,no wound in fection and complication in follow-up period of six months,the neurological function in the tuberculosis patients were improved. Conclusion:process of upper spine was exposed very clearly and the operation field was widened with this approach,enough resection was available and spine sequence was improved.the surgical approach can provide enough space for anterior decompression and implant with few complications.It may be an ideal approach to treat disorders of the lesion of upper thoracic spine.
