"First Treating, Post-paid" Model Studies

"Difficult and expensive of medical treatment" and the doctor-patient-relationship disharmony are the main outstanding and contradiction problems in our health services. Our Party and the government also highly concerned about people’s livelihood, they do their utmost to solve it actively. Core of the new health care reform in2009is to highlight the public welfare health services,"first treating, post-paid" model which is the most popular news and aroused widespread in2013. The model open a "green channel of life" for each patient, so patients will not be raised because of insufficient medical expenses due to delay treatment period. The public is keen to implement the model as soon as possible; on the contrary, the Ministry of Health held a press conference to clarify the current condition is not yet ripe.Medical treatment as a social-public fields take on the social responsibility to provide public health services."First treating, post-paid" model from the entire society section, will help alleviate the "difficult and expensive of medical treatment" problem; raise the level of credit of the whole society, to promote spiritual civilization; promote China’s new medical reform, give impetus to medical-aid system’s development. From the individual’s point of view,"first treating, post-paid" model guarantee universal access to basic health services, insure personal treating in time; reduce patient disputes phenomenon. People will get the benefits, as well as the public hospital from this system."First treating, post-paid" model has been launched in Jining, one of city in Shandong Province. The model has been extended to262medical institutions; the total beneficiaries reached64million people, at all levels of medical institutions to advance medical funds1.674billion Yuan by the end of2013in Jining. Therefore, it has practical significance and necessity to implement the model.This article is based mainly on theoretical study, combined by the method of reference literature research, case studies and comparative analysis. By referring to relevant literature,books for data compilation, provide a theoretical basis for the article writing; the article’s dates come from Ministry of statistic, Ministry of Health, Ministry of Finance, Ministry of Human Resources and Social Security, and get the related knowledge from the websites; through online questionnaires and surveys of the Health Department of Shandong Province, Jining Medical insurance agencies to collect some related information concerned about "first treating, post-paid " model to provide a realistic basis for the article writing. At this stage, public hospitals are also the subject of the health care system, so this article focuses on the impact of the implementation of the model to public hospitals, including the positive and negative aspects. The article through five chapters to illustrate the necessity of the "first treating, post-paid" model and the impacts to public hospitals:The first part is an introduction, describes the background and significance of this paper, the related field’s researches at home and abroad in current condition, innovation of this writing, research ideas and methods as well as deficiencies; The second part describes the "first treating, post-paid " model-theory, including medical equitable theory, concept of two paid models and specific operating procedures, implement the model’s necessary as well as the need for two operation mode of public hospital funding under the two models; The third part is a key chapter of this paper describes the status of the model and the problems in Shandong province, and analysis the impacts of the implementation of the model for public hospitals in Shandong Province under research, we also analysis the reason aim to solve the problems; The fourth part describes the typical foreign countries and China Taiwan region’s medical paid model, and analysis advanced and scientific areas of them, combined with the specific situation of public hospitals, presented the inspirations of our health care system and the revelation pay model; The fifth part is a key chapter of this paper, according to our hospitals status and foreign advised experiences to make concrete proposals for "first treating, post-paid" At the important stage of public hospitals’ reform "first treating, post-paid" model support a new opinions to promote the development of China’s medical insurance system and public hospitals.At this stage, my knowledge, vision and theoretical knowledge is limited, some of the views or references immaturity inevitably exist, and need to study and work in the future to continue the research. In addition, the article is an empirical analysis of pay model public hospitals aiming to implementation of Jining City Shandong Province.Besides the foreigner scholars don’t spend too much time on "first treating, past-paid research,it is difficult for me to study and collection of information. As a result, the resource of data and collection of information are not much richer enough. So, I have to endeavor and improve indelibly.
